Indications

  • Treatment of infections caused by sensitive Gram-positive and Gram-negative bacteria
  • Upper respiratory tract infections: sinusitis, pharyngitis, tonsillitis, laryngo-tracheo bronchitis, otitis media
  • Lower respiratory tract infections: bronchitis, lobar pneumonia, bronchopneumonia
  • Urinary tract infections: cystitis, urethritis, pyelonephritis
  • Skin and soft tissue infections: abscess, cellulitis, furunculosis, impetigo

Pharmacology

  • Semisynthetic broad spectrum bactericidal antibiotic
  • Active against both gram-positive and gram-negative microorganisms
  • Inhibits cross-linking process of peptidoglycan in bacteria cell wall
  • Effective against both penicillinase producing and nonproducing staphylococci

Dosage & Administration

  • For oral administration in adults and children
  • Normal daily doses for different types of infections
  • Maximum daily dosage: 4 gm
  • Injectable administration dosage for adults and children

Interaction

  • Increased risk of kidney damage with concomitant use of nephrotoxic drugs such as aminoglycosides

Contraindications

  • Should not be used in patients with known or suspected hypersensitivity to cephalosporins

  • Reversible interstitial nephritis

Pregnancy & Lactation

  • Safety in pregnancy not established
  • Excreted in breast milk, caution in lactating mothers

Precautions & Warnings

  • Prolonged use may result in the development of superinfection
  • Administered with care to patients hypersensitive to penicillins
  • May cause a false positive urine glucose result
  • Dosage adjustment is necessary in renal impairment
  • Contains lactose, caution for patients with rare hereditary problems of galactose intolerance

Use in Special Populations

  • Recommended doses for renal impairment
  • Higher doses may be required for chronic or severe infections
  • Paediatric dosages should not exceed those specified for adults
  • Regular bacteriological and clinical monitoring required for chronic urinary tract infections

Overdose Effects

  • Non-specific symptoms: nausea, vomiting, diarrhoea, gastric upsets
  • Mainly supportive treatment with gastric lavage if large amount ingested

Storage Conditions

  • Extracef DS Suspension should be freshly prepared
  • Reconstituted Suspension should be used within 7 days at room temperature or within 14 days in a refrigerator
  • Extracef DS Injection solutions should be used within 2 hours at room temperature
  • Retain potency for 12 hours when stored at 5°C
  • Reconstituted solutions may vary in colour but it does not affect potency
  • Keep all medicines out of the reach of children
  • To be dispensed only on the prescription of a registered physician
