Indications
- Treatment of infections caused by sensitive Gram-positive and Gram-negative bacteria
- Upper respiratory tract infections: sinusitis, pharyngitis, tonsillitis, laryngo-tracheo bronchitis, otitis media
- Lower respiratory tract infections: bronchitis, lobar pneumonia, bronchopneumonia
- Urinary tract infections: cystitis, urethritis, pyelonephritis
- Skin and soft tissue infections: abscess, cellulitis, furunculosis, impetigo
Pharmacology
- Semisynthetic broad spectrum bactericidal antibiotic
- Active against both gram-positive and gram-negative microorganisms
- Inhibits cross-linking process of peptidoglycan in bacteria cell wall
- Effective against both penicillinase producing and nonproducing staphylococci
Dosage & Administration
- For oral administration in adults and children
- Normal daily doses for different types of infections
- Maximum daily dosage: 4 gm
- Injectable administration dosage for adults and children
Interaction
- Increased risk of kidney damage with concomitant use of nephrotoxic drugs such as aminoglycosides
Contraindications
- Should not be used in patients with known or suspected hypersensitivity to cephalosporins

- Blood disorders, immune system disorders, psychiatric disorders, nervous system disorders
- Hepatobiliary disorders, renal and urinary disorders
- Elevation of blood urea nitrogen, serum creatinine, liver enzyme disturbances
- Reversible interstitial nephritis
Pregnancy & Lactation
- Safety in pregnancy not established
- Excreted in breast milk, caution in lactating mothers
Precautions & Warnings
- Prolonged use may result in the development of superinfection
- Administered with care to patients hypersensitive to penicillins
- May cause a false positive urine glucose result
- Dosage adjustment is necessary in renal impairment
- Contains lactose, caution for patients with rare hereditary problems of galactose intolerance
Use in Special Populations
- Recommended doses for renal impairment
- Higher doses may be required for chronic or severe infections
- Paediatric dosages should not exceed those specified for adults
- Regular bacteriological and clinical monitoring required for chronic urinary tract infections
Overdose Effects
- Non-specific symptoms: nausea, vomiting, diarrhoea, gastric upsets
- Mainly supportive treatment with gastric lavage if large amount ingested

Storage Conditions
- Extracef DS Suspension should be freshly prepared
- Reconstituted Suspension should be used within 7 days at room temperature or within 14 days in a refrigerator
- Extracef DS Injection solutions should be used within 2 hours at room temperature
- Retain potency for 12 hours when stored at 5°C
- Reconstituted solutions may vary in colour but it does not affect potency
- Keep all medicines out of the reach of children
- To be dispensed only on the prescription of a registered physician
