Indications

  • Essential hypertension

Pharmacology

  • Dihydropyridine calcium-channel blocker
  • Long duration of action
  • Vasodilation
  • Reduced peripheral resistance
  • Increased coronary blood flow
  • Reflex increase in coronary heart rate

Dosage & Administration

  • Usual dose is 5 mg once daily for hypertension
  • Maximum dose is 10 mg once daily for hypertension
  • 5 to 10 mg for angina
  • May be taken without regard to meals

Interaction

  • Potentially hazardous interactions with p-blockers
  • No interaction between G-Amlo and Digoxin
  • No interaction between G-Amlo and Cimetidine
  • G-Amlo did not significantly alter the effect of Warfarin on prothrombin time
  • Food does not alter the rate or extent of absorption of G-Amlo

Contraindications

  • Hypersensitivity to dihydropyridine derivatives
  • Pregnancy

Pregnancy & Lactation

  • Pregnancy Category C
  • Amlodipine should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus
  • It is recommended that nursing be discontinued while Amlodipine is administered

Precautions & Warnings

  • Precaution should be taken in patients with hepatic impairment and during pregnancy and breastfeeding

Use in Special Populations

  • Specific dosage for children with hypertension from 6 years to 17 years of age
  • Effect of amlodipine on blood pressure in patients less than 6 years of age is not known
  • Elderly patients can use G-Amlo at similar doses as younger patients
  • Normal dosage regimens are recommended in the elderly
  • Dosage recommendations in patients with mild to moderate hepatic impairment
  • Normal dosage recommended in renal impairment
  • Dosage initiation at the lowest dose in severe hepatic impairment

Overdose Effects

  • Excessive peripheral vasodilatation
  • Reflex tachycardia
  • Marked and prolonged systemic hypotension
  • Active cardiovascular support required
  • Vasoconstrictor and intravenous calcium gluconate may be beneficial
  • Gastric lavage may be worthwhile in some cases
  • Charcoal use can reduce absorption rate of amlodipine
  • Dialysis is not likely to be of benefit
