Pharmacology

Dosage & Administration

  • Usual dose of 5 mg once daily for hypertension
  • Maximum dose of 10 mg once daily for hypertension
  • Elderly patients with hepatic insufficiency may start on 2.5 mg once daily
  • Dose of 5 to 10 mg for angina
  • Lower dose for elderly and patients with hepatic insufficiency

Interaction

  • Potentially hazardous interactions with p-blockers
  • No interaction between Hipre and Digoxin
  • No interaction between Hipre and Cimetidine
  • No significant alteration of Warfarin effect by Hipre
  • Food does not alter the rate or extent of absorption of Hipre

Contraindications

  • Hypersensitivity to dihydropyridine derivatives
  • Pregnancy

Pregnancy & Lactation

  • Adequate and well-controlled studies lacking
  • Recommendation to discontinue nursing while Amlodipine is administered

  • Precaution in patients with hepatic impairment
  • Precaution during pregnancy and breastfeeding

Use in Special Populations

  • Dosing recommendations for children with hypertension
  • No known effect of amlodipine on blood pressure in patients under 6 years old
  • Similar toleration in elderly and younger patients
  • Normal dosage recommended for renal impairment
  • Dosing caution for mild to moderate hepatic impairment

Overdose Effects

  • Excessive peripheral vasodilatation
  • Reflex tachycardia
  • Marked and prolonged systemic hypotension
  • Active cardiovascular support required
  • Potential use of vasoconstrictor
  • Intravenous calcium gluconate may be beneficial
  • Potential use of gastric lavage
