Indications

  • Treatment of infections caused by sensitive Gram-positive and Gram-negative bacteria
  • Upper respiratory tract infections: sinusitis, pharyngitis, tonsillitis, laryngo-tracheo bronchitis, otitis media
  • Lower respiratory tract infections: bronchitis, lobar pneumonia, bronchopneumonia
  • Urinary tract infections: cystitis, urethritis, pyelonephritis
  • Skin and soft tissue infections: abscess, cellulitis, furunculosis, impetigo

Pharmacology

  • Semisynthetic broad spectrum bactericidal antibiotic
  • Active against both gram-positive and gram-negative microorganisms
  • Inhibits cross-linking process in bacterial cell wall
  • Lysis of bacteria due to external osmotic pressure

Dosage & Administration

  • For oral administration in adults and children
  • Injectable administration for adults and children
  • Dosage adjustment in renal impairment
  • Maximum daily dosage: 4 gm
  • Precaution for patients with impaired renal or hepatic function

Interaction

  • Concomitant use of nephrotoxic drugs may increase the risk of kidney damage
  • Diuretics and probenecid enhance possibility of renal toxicity

Contraindications

  • Not to be used in patients with known or suspected hypersensitivity to cephalosporins

Side Effects

  • Limited essentially to gastro-intestinal disturbances
  • Hypersensitivity phenomena may occur
  • Possible skin reactions
  • Rare: glossitis, heartburn, dizziness, nausea, vomiting, diarrhoea, abdominal pain
  • Other rare effects on blood, immune, psychiatric, nervous, hepatobiliary, renal, and urinary systems

Pregnancy & Lactation

  • Safety in pregnancy not established
  • Excreted in breast milk, caution in lactating mothers
  • Patients cautioned about operating hazardous machinery

Precautions & Warnings

  • Prolonged use may result in superinfection
  • Care in administering to patients hypersensitive to penicillins
  • May cause false positive urine glucose result with certain tests
  • Dosage adjustment necessary in renal impairment
  • Contains lactose, caution for patients with rare hereditary problems

Use in Special Populations

  • Recommended doses for patients with renal impairment
  • Additional information for all patients regardless of age or weight
  • Treatment duration for various infections
  • Paediatric dosages not to exceed those specified for adults

Overdose Effects

  • Non-specific symptoms: nausea, vomiting, diarrhoea, gastric upsets
  • Treatment mainly supportive
  • Gastric lavage necessary for large ingestions
