Indications
- Essential hypertension
- Angina pectoris
- Vasospastic angina
Pharmacology
- Dihydropyridine calcium-channel blocker
- Long duration of action
- Vasodilation
- Reduced peripheral resistance
- Increased coronary blood flow
Dosage & Administration
- Usual dose is 5 mg once daily for hypertension
- Maximum dose is 10 mg once daily for hypertension
- Lower dose for elderly and hepatic insufficiency
- May be taken without regard to meals
Interaction
- Potentially hazardous interactions with p-blockers in patients with impaired cardiac function
- No interaction with Digoxin
- No interaction with Cimetidine
- No significant alteration of Warfarin effect on prothrombin time
- Food does not alter absorption
Contraindications
- Hypersensitivity to dihydropyridine derivatives
- Pregnant women

- Dizziness
- Flushing
- Headache
- Hypotension
- Peripheral edema
- Gastrointestinal disturbances
- Increased micturition frequency
- Lethargy
- Eye pain
- Mental depression
- Ischaemic chest pain
- Excessive fall in blood pressure
- Rashes
- Fever
- Abnormalities in liver function
Pregnancy & Lactation
- Pregnancy Category C
- No adequate studies on pregnant women
- Use only if benefits justify potential risks
- Not known if excreted in human milk
- Recommended to discontinue nursing while administered
Precautions & Warnings
- Care in patients with hepatic impairment
- Care during pregnancy and breastfeeding
Use in Special Populations
- Dosing for children with hypertension
- Effect on blood pressure in children under 6 years not known
- Similar tolerance in elderly and younger patients
- Normal dosage recommended for renal impairment
- Dose selection should be cautious for hepatic impairment
Overdose Effects
- Excessive peripheral vasodilatation
- Reflex tachycardia
- Marked and prolonged systemic hypotension
- Active cardiovascular support required
- Monitoring of cardiac and respiratory function
- Elevation of extremities
- Attention to fluid volume and urine output
- Vasoconstrictor may be helpful
- Intravenous calcium gluconate may be beneficial
- Gastric lavage may be worthwhile
- Use of charcoal up to 2 hours after administration
- Dialysis is unlikely to be beneficial
