Indications

Pharmacology

  • Calcium-channel blocker
  • Vasodilation
  • Reduced peripheral resistance
  • Increased coronary blood flow
  • Myocardial oxygen supply
  • Cardiac output

Dosage & Administration

  • Hypertension: Usual dose is 5 mg once daily
  • Maximum dose is 10 mg once daily
  • Angina: 5 to 10 mg, lower dose for elderly and patients with hepatic insufficiency
  • May be taken without regard to meals

Interaction

  • Potentially hazardous interactions with p-blockers
  • No interaction between Lodipin and Digoxin in healthy volunteers
  • No interaction between, Lodipin and Cimetidine in healthy volunteers
  • No significant alteration of Warfarin effect on prothrombin time by Lodipin
  • Food does not alter the rate or extent of absorption of Lodipin

Contraindications

  • Hypersensitivity to dihydropyridine derivatives
  • Pregnant woman

Pregnancy & Lactation

  • Pregnancy Category C
  • Use during pregnancy only if potential benefit justifies potential risk to the fetus
  • Recommendation to discontinue nursing while Amlodipine is administered

Precautions & Warnings

  • Precaution in patients with hepatic impairment
  • Precaution during pregnancy and breast feeding

Use in Special Populations

  • Children with hypertension from 6 years to 17 years of age
  • Children under 6 years old
  • Elderly patients
  • Renal impairment
  • Hepatic impairment

Overdose Effects

  • Excessive peripheral vasodilatation
  • Reflex tachycardia
  • Marked and prolonged systemic hypotension up to shock
  • Management: active cardiovascular support, monitoring of cardiac and respiratory function, elevation of extremities, attention to circulating fluid volume and urine output, vasoconstrictor, intravenous calcium gluconate, gastric lavage, use of charcoal
  • Dialysis is not likely to be of benefit
