Indications
- Treatment of essential hypertension
- Treatment of angina pectoris
- Treatment of vasospastic angina
- Efficacious as monotherapy in hypertension
- May be used in combination with other antihypertensive agents
- Efficacious as monotherapy in chronic stable angina pectoris
- May be used in combination with other antianginal agents
Pharmacology
- Dihydropyridine calcium-channel blocker
- Long duration of action
- Influences myocardial cells and cells of vascular smooth muscle
- Results in vasodilation and reduced peripheral resistance
- Increases coronary blood flow and reflex increase in coronary heart rate
- Increases myocardial oxygen supply and cardiac output
Dosage & Administration
- Usual dose for hypertension: 5 mg once daily
- Maximum dose for hypertension: 10 mg once daily
- Usual dose for angina: 5-10 mg once daily
- Lower dose for elderly and patients with hepatic insufficiency
- May be taken without regard to meals
Interaction
- Potentially hazardous interactions with p-blockers in patients with severely impaired cardiac left ventricular function
- No interaction between Lopin and Digoxin in healthy volunteers
- No interaction between Lopin and Cimetidine in healthy volunteers
- No significant alteration of Warfarin effect on prothrombin time by Lopin
- Food does not alter absorption rate of Lopin
Contraindications
- Hypersensitivity to dihydropyridine derivatives
- Pregnancy
Side Effects
- Common adverse effects: dizziness, flushing, headache, hypotension, peripheral edema
- Other effects: gastrointestinal disturbances, increased micturition frequency, lethargy, eye pain, mental depression, paradoxical increase in ischaemic chest pain
Pregnancy & Lactation
- Pregnancy Category C
- Use during pregnancy only if potential benefit justifies potential risk to fetus
- Recommendation to discontinue nursing while Amlodipine is administered due to unknown excretion in human milk
Precautions & Warnings
- Precaution in patients with hepatic impairment
- Precaution during pregnancy and breastfeeding
Use in Special Populations
- Dosing for children with hypertension from 6 to 17 years: 2.5 mg once daily, up-titrated to 5 mg once daily if necessary
- No studied dosages in excess of 5 mg daily in pediatric patients
- Effect on blood pressure in patients under 6 years old not known
- Lopin used at similar doses in elderly or younger patients is equally well tolerated
- Normal dosage recommended in the elderly
- Careful dosage selection in patients with mild to moderate hepatic impairment
- Initiate Lopin at lowest dose in severe hepatic impairment
Overdose Effects
- Symptoms: excessive peripheral vasodilatation, reflex tachycardia, systemic hypotension, shock
- Management: active cardiovascular support, monitoring of cardiac and respiratory function, elevation of extremities, attention to circulating fluid volume and urine output, use of vasoconstrictor, intravenous calcium gluconate, potential gastric lavage
