Medicine Details

Indications

  • Treatment of infections caused by sensitive Gram-positive and Gram-negative bacteria
  • Upper respiratory tract infections: sinusitis, pharyngitis, tonsillitis, laryngo-tracheo bronchitis, otitis media
  • Lower respiratory tract infections: bronchitis (acute and chronic), lobar pneumonia, bronchopneumonia
  • Urinary tract infections: cystitis, urethritis, pyelonephritis
  • Skin and soft tissue infections: abscess, cellulitis, furunculosis, impetigo

Pharmacology

  • Semisynthetic broad spectrum bactericidal antibiotic
  • Active against both gram-positive and gram-negative microorganisms
  • Inhibits cross-linking process of cell wall of bacteria

Dosage & Administration

  • For oral administration in adults and children
  • For injectable administration in adults and children
  • Dosage adjustments for renal impairment
  • Maximum daily dosage: 4 gm
  • Preoperative prophylaxis dosage

Interaction

  • Increased risk of kidney damage with concomitant use of nephrotoxic drugs
  • Enhanced renal toxicity with diuretics and probenecid

Contraindications

  • Not for use in patients with known or suspected hypersensitivity to cephalosporins

  • Gastro-intestinal disturbances
  • Hypersensitivity phenomena
  • Possible skin reactions
  • Rare: Glossitis, heartburn, dizziness, nausea, vomiting, diarrhoea, abdominal pain, vaginitis, candida overgrowth, urticaria, joint pains, oedema, blood disorders, fever, serum sickness like reactions, anaphylaxis, confusion, sleep disturbances, liver enzyme disturbances, transient hepatitis, cholestatic jaundice, reversible interstitial nephritis

Pregnancy & Lactation

  • No established safety in pregnancy
  • Excreted in breast milk
  • Caution in lactating mothers

Precautions & Warnings

  • Prolonged use may result in the development of superinfection
  • Cross-sensitivity risk in patients hypersensitive to penicillins
  • Potential positive Coombs’ testing result
  • False positive urine glucose result with certain tests
  • Dosage adjustment for renal impairment
  • Contains lactose

Use in Special Populations

  • Renal impairment dosing recommendations
  • Higher doses required for chronic or severe infections
  • Duration of treatment for infections with haemolytic streptococci
  • Regular bacteriological and clinical monitoring for chronic urinary tract infections

Overdose Effects

  • Non-specific symptoms: nausea, vomiting, diarrhoea, gastric upsets
  • Mainly supportive treatment
  • Gastric lavage if large amount ingested
