Indications

  • Essential hypertension
  • Angina pectoris
  • Vasospastic angina
  • Monotherapy in hypertension treatment
  • Monotherapy in chronic stable angina pectoris treatment
  • Vasospastic angina treatment
  • Combination therapy with other antihypertensive agents
  • Combination therapy with other antianginal agents

Pharmacology

  • Dihydropyridine calcium-channel blocker
  • Long duration of action
  • Influences myocardial cells
  • Influences specialized conducting system of the heart
  • Influences vascular smooth muscle
  • Results in vasodilation
  • Reduces peripheral resistance
  • Increases coronary blood flow
  • Increases myocardial oxygen supply
  • Increases cardiac output

Dosage & Administration

  • Usual dose: 5 mg once daily for hypertension
  • Usual dose: 5 to 10 mg for angina
  • Maximum dose: 10 mg once daily
  • Starting dose for elderly with hepatic insufficiency: 2.5 mg once daily
  • May be taken without regard to meals

Interaction

  • Potentially hazardous interactions with p-blockers in patients with impaired cardiac function
  • No interaction with Digoxin
  • No interaction with Cimetidine
  • No significant alteration of Warfarin effect on prothrombin time
  • Food does not alter absorption rate

Contraindications

  • Hypersensitivity to dihydropyridine derivatives
  • Pregnancy

Side Effects

  • Dizziness
  • Flushing
  • Headache
  • Hypotension
  • Peripheral edema
  • Gastrointestinal disturbances
  • Increased micturition frequency
  • Lethargy
  • Eye pain
  • Mental depression
  • Excessive fall in blood pressure
  • Ischaemic chest pain at start of treatment
  • Rashes
  • Fever
  • Abnormalities in liver function

Pregnancy & Lactation

  • Pregnancy Category C
  • No adequate and well-controlled studies in pregnant women
  • Should be used during pregnancy if benefit justifies risk to fetus
  • Not known whether excreted in human milk
  • Discontinuation of nursing while administered

Precautions & Warnings

  • Should be taken in patients with hepatic impairment
  • Precaution during pregnancy and breastfeeding

Use in Special Populations

  • Pediatric dosing for hypertension
  • Effect on blood pressure in children under 6 years is not known
  • Elderly tolerance similar to younger patients
  • Normal dosage recommended for renal impairment
  • Dosing caution in mild to moderate hepatic impairment
  • Initiate at lowest dose for severe hepatic impairment

Overdose Effects

  • Excessive peripheral vasodilatation
  • Reflex tachycardia
  • Marked and prolonged systemic hypotension
  • Management for hypotension due to overdose
