Medicine Details

Indications

  • Treatment of infections caused by sensitive Gram-positive and Gram-negative bacteria
  • Upper respiratory tract infections: sinusitis, pharyngitis, tonsillitis, laryngo-tracheo bronchitis, otitis media
  • Lower respiratory tract infections: bronchitis (acute and chronic), lobar pneumonia, bronchopneumonia
  • Urinary tract infections: cystitis, urethritis, pyelonephritis
  • Skin and soft tissue infections: abscess, cellulitis, furunculosis, impetigo

Pharmacology

  • Broad spectrum bactericidal antibiotic
  • Active against both gram-positive and gram-negative microorganisms
  • Inhibits cross-linking process of bacterial cell wall

Dosage & Administration

  • Oral and injectable administration
  • Adult dosage for urinary tract infections, respiratory tract infections, and skin and soft tissue infections
  • Children's dosage for otitis media
  • Elderly and impaired renal or hepatic function monitoring

Interaction

  • Nephrotoxic drug interaction
  • Enhanced risk of renal toxicity with diuretics and probenecid

Contraindications

  • Hypersensitivity to cephalosporins

Side Effects

  • Gastro-intestinal disturbances
  • Hypersensitivity phenomena
  • Skin reactions, glossitis, dizziness, nausea, vomiting, diarrhoea, abdominal pain, vaginitis, candida overgrowth
  • Rare blood, immune, psychiatric, nervous system, hepatobiliary, renal, and urinary disorders
  • Elevation of blood urea nitrogen, serum creatinine, liver enzyme disturbances

Pregnancy & Lactation

  • Safety not established in pregnancy
  • Excreted in breast milk, caution in lactating mothers

Precautions & Warnings

  • Risk of superinfection
  • Administration with care to penicillin-hypersensitive patients
  • Positive Coombs’ testing
  • Causes false positive urine glucose result with specific testing
  • Dosage adjustment in renal impairment
  • Contains lactose, caution in patients with rare hereditary problems

Use in Special Populations

  • Recommended doses for patients with renal impairment and haemodialysis
  • Higher doses for chronic or severe infections
  • Regular bacteriological and clinical monitoring for urinary tract infections
  • No exceeding of paediatric dosages

Overdose Effects

  • Non-specific symptoms: nausea, vomiting, diarrhoea, gastric upsets
  • Supportive treatment, gastric lavage if necessary
