Indications
- Essential hypertension
- Angina pectoris
- Vasospastic angina
Pharmacology
- Dihydropyridine calcium-channel blocker
- Long duration of action
- Vasodilation
- Reduced peripheral resistance
- Increased coronary blood flow
- Reflex increase in coronary heart rate
Dosage & Administration
- Usual dose is 5 mg once daily for hypertension
- Maximum dose is 10 mg once daily for hypertension
- Usual dose is 5 to 10 mg for angina
- May be taken without regard to meals
Interaction
- Potentially hazardous interactions with p-blockers
- No interaction between Nelod and Digoxin
- No interaction between Nelod and Cimetidine
- Nelod did not significantly alter the effect of Warfarin on prothrombin time
- Food does not alter the rate or extent of absorption of Nelod
Contraindications
- Hypersensitivity to dihydropyridine derivatives
- Pregnant woman
Side Effects
- Dizziness
- Flushing
- Headache
- Hypotension
- Peripheral edema
- Gastrointestinal disturbances
- Increased micturition frequency
- Lethargy
- Eye pain
- Mental depression
- Ischaemic chest pain
- Excessive fall in blood pressure
- Rashes
- Fever
- Abnormalities in liver function
Pregnancy & Lactation
- Pregnancy Category C
- Use during pregnancy only if potential benefit justifies potential risk to the fetus
- Amlodipine is not known to be excreted in human milk
- Recommended that nursing be discontinued while Amlodipine is administered
Precautions & Warnings
- Precaution in patients with hepatic impairment
- Precaution during pregnancy and breastfeeding
Use in Special Populations
- Dosing recommendations for children with hypertension
- Effect of amlodipine on blood pressure in patients less than 6 years of age is not known
- Similar dose tolerance in elderly and younger patients
- Normal dosage recommended for renal impairment
- Dosage recommendations not established for hepatic impairment
- Nelod is not dialysable
Overdose Effects
- Excessive peripheral vasodilatation
- Reflex tachycardia
- Marked and prolonged systemic hypotension
- Active cardiovascular support needed for clinically significant hypotension
- Intravenous calcium gluconate may be beneficial
- Gastric lavage may be worthwhile in some cases
- Charcoal may reduce the absorption rate of amlodipine
- Dialysis is not likely to be of benefit due to high protein-binding
