Indications

  • Treatment of infections caused by sensitive Gram-positive and Gram-negative bacteria
  • Upper respiratory tract infections
  • Lower respiratory tract infections
  • Urinary tract infections
  • Skin and soft tissue infections

Pharmacology

  • Bactericidal antibiotic
  • Active against gram-positive and gram-negative microorganisms
  • Inhibition of cross-linking process in the cell wall of bacteria
  • Cell wall lysis due to external osmotic pressure

Dosage & Administration

  • For oral administration - Adults:
    • Urinary tract infections: 500mg four times daily or 1g twice daily
    • Respiratory tract infections: 250 to 500mg four times daily or 500mg to 1g twice daily
    • Skin and soft tissue infections: 250 to 500mg four times daily or 500mg to 1g twice daily
  • For oral administration - Children:
    • Total daily dose of 25 to 50mg/kg given in two or four equally divided doses
    • Otitis media: Total daily dose of 75 to 100mg/kg given in divided doses 6 to 12 hourly
  • For injectable administration - Adult: 2-4 gm daily in four equally divided doses up to 8 gm daily
  • For injectable administration - Children: 50-100 mg/kg daily in four equally divided doses, up to 300 mg/kg daily in severe infection

Interaction

  • Potential risk of kidney damage when used concomitantly with nephrotoxic drugs
  • Enhanced possibility of renal toxicity with diuretics and probenecid

Contraindications

Not to be used in patients with known or suspected hypersensitivity to cephalosporins

Precautions & Warnings

  • Prolonged use may result in superinfection
  • Administer with care to patients hypersensitive to penicillins
  • May cause a false positive urine glucose result
  • Dosage adjustment necessary in renal impairment
  • Contains lactose - caution in patients with rare hereditary problems
  • Renal Impairment: Recommended doses based on creatinine clearance levels
  • Paediatric dosages should not exceed those specified for adults

Use in Special Populations

Recommended doses for patients with renal impairment and on chronic, intermittent haemodialysis

Overdose Effects

Non-specific symptoms such as nausea, vomiting, and diarrhoea
