Indications
- Treatment of infections caused by sensitive Gram-positive and Gram-negative bacteria
- Upper respiratory tract infections
- Lower respiratory tract infections
- Urinary tract infections
- Skin and soft tissue infections
Pharmacology
- Broad spectrum bactericidal antibiotic
- Active against gram-positive and gram-negative microorganisms
- Inhibition of cross-linking process in cell wall
- Resulting in lysis of bacteria due to external osmotic pressure
Dosage & Administration
- For oral administration - Adults:
- 500mg four times daily or 1g twice daily for urinary tract infections
- 250 to 500mg four times daily or 500mg to 1g twice daily for respiratory tract infections
- 250 to 500mg four times daily or 500mg to 1g twice daily for skin and soft tissue infections
- Maximum daily dosage: 4 gm
- For oral administration - Children:
- Total daily dose of 25 to 50mg/kg
- Maximum daily dosage: 4 gm
- For injectable administration - Adult:
- 2-4 gm daily in four equally divided doses up to 8 gm daily
- 1-2 gm prophylactic dose
- For injectable administration - Children:
- 50-100 mg/kg daily in four equally divided doses up to 300 mg/kg daily
Interaction
- Increased risk of kidney damage with nephrotoxic drugs
- Enhanced renal toxicity with diuretics and probenecid
Contraindications
- Known or suspected hypersensitivity to cephalosporins
Side Effects
- Gastro-intestinal disturbances: Limited essentially to gastrointestinal disturbances
- Hypersensitivity phenomena: More likely in individuals with previous hypersensitivity
- Rare side effects:
- Glossitis
- Heartburn
- Dizziness
- Nausea
- Vomiting
- Diarrhoea
- Abdominal pain
- Vaginitis
- Candida overgrowth
- Skin reactions
- Joint pains
- Oedema
- Other disorders:
- Blood disorders
- Fever
- Psychiatric disorders
- Liver related disorders
- Reversible interstitial nephritis
- Elevation of certain blood parameters
Pregnancy & Lactation
- Not established safety in pregnancy
- Excreted in breast milk, caution in lactating mothers
Precautions & Warnings
- Prolonged use: May result in development of superinfection
- Cross-sensitivity: Care in patients hypersensitive to penicillins
- False positive urine glucose result: May occur in testing
- Dosage adjustment: Necessary in renal impairment
- Lactose intolerance: Should not be taken by patients with rare hereditary problems
Use in Special Populations
- Recommended doses for renal impairment
- Higher doses may be required for chronic or severe infections
- Regular bacteriological and clinical monitoring for chronic urinary tract infections
- Dosages should not exceed those specified for adults in pediatric use
Overdose Effects
- Non-specific symptoms such as nausea, vomiting, and diarrhea
- Supportive treatment
- Gastric lavage if large amount ingested

Storage Conditions
- Reconstituted Suspension should be used within 7-14 days
- Solutions retain potency for 12 hours when stored at 5°C
- Do not use later than the date of expiry
- To be dispensed only on the prescription of a registered physician
