Pharmacological Action

  • Bactericidal
  • Inhibits cell wall cross-linking
  • Active against gram-positive and gram-negative microorganisms
  • Effective against penicillinase producing and non-producing staphylococci

Dosage and Administration

  • 500mg four times daily for urinary tract infections (adults)
  • 1g twice daily for urinary tract infections (adults)
  • 250 to 500mg four times daily for respiratory tract infections (adults)
  • 500mg to 1g twice daily for respiratory tract infections (adults)
  • 250 to 500mg four times daily for skin and soft tissue infections (adults)
  • 500mg to 1g twice daily for skin and soft tissue infections (adults)
  • 25 to 50mg/kg total daily dose for children
  • Maximum daily dosage of 4 gm
  • 2-4 gm daily in four equally divided doses for adults (injectable administration)
  • 50-100 mg/kg daily in four equally divided doses for children (injectable administration)

Interaction

Nephrotoxic drugs may increase risk of kidney damage when used concomitantly

Pregnancy and Lactation

Precautions and Warnings

  • Risk of superinfection with prolonged use
  • Careful administration to patients hypersensitive to penicillins
  • Potential false positive urine glucose result
  • Dosage adjustment required in renal impairment
  • Contains lactose (avoid in hereditary lactase deficiencies)

Use in Special Populations

  • Recommended doses for renal impairment
  • Caution in patients on haemodialysis
  • Higher doses may be required for chronic or severe infections
  • Regular bacteriological and clinical monitoring for chronic urinary tract infections
  • Paediatric dosages should not exceed those specified for adults

Overdose Effects

Non-specific symptoms: nausea, vomiting, diarrhoea, gastric upsets
