Indications

  • Treats infections caused by sensitive Gram-positive and Gram-negative bacteria
  • Effective for upper respiratory tract infections, lower respiratory tract infections, urinary tract infections, skin and soft tissue infections

Pharmacology

  • Semisynthetic broad spectrum bactericidal antibiotic
  • Active against both gram-positive and gram-negative microorganisms
  • Inhibits cross-linking process in cell wall of bacteria

Dosage & Administration

  • Adult oral administration: 500mg-1g four times daily for urinary tract infections
  • Adult oral administration: 250-500mg four times daily for respiratory tract infections
  • Children oral administration: 25-50mg/kg daily divided doses
  • Adult injectable administration: 2-4g daily in four equally divided doses

Interaction

  • Concomitant use with nephrotoxic drugs may increase risk of kidney damage
  • Diuretics and probenecid enhance possibility of renal toxicity

Contraindications

  • Not to be used in patients with known or suspected hypersensitivity to cephalosporins

Side Effects

  • Primarily gastrointestinal disturbances
  • Possible hypersensitivity reactions including skin rashes and joint pains
  • Rare blood disorders, immune system disorders, psychiatric disorders, nervous system disorders, hepatobiliary disorders, renal and urinary disorders

Pregnancy & Lactation

  • Safety in pregnancy not established
  • Excreted in breast milk, caution in lactating mothers

Precautions & Warnings

  • Prolonged use may result in superinfection
  • Administer with care to patients hypersensitive to penicillins
  • May cause false positive urine glucose result with certain tests

Use in Special Populations

  • Dose adjustments for renal impairment
  • Higher doses may be required for chronic or severe infections
  • Regular bacteriological and clinical monitoring required for chronic urinary tract infections

Overdose Effects

  • Non-specific symptoms include nausea, vomiting, diarrhea and gastric upsets
  • Treatment is mainly supportive, gastric lavage may be necessary if large amount ingested

  • Suspension should be freshly prepared and used within specified time frames
