Pharmacology

  • Stability against b-lactamase inactivation
  • Broad spectrum of activity
  • Active against various organisms in vitro

Dosage & Administration

  • Adult dose: 250 mg every 8 hours
  • Maximum adult dosage: 4 g/day
  • Pediatric dose: 20 mg/kg/day in divided doses every 8 hours
  • Renal impairment dosage adjustments not usually required
  • Haemodialysis dosage recommendations

Interaction

  • False-positive reaction for glucose in the urine
  • Reports of increased anticoagulant effect
  • Regular monitoring of prothrombin time recommended

Contraindications

  • Known allergy to Cephalosporin group of antibiotics

Side Effects

  • Gastro-intestinal symptoms
  • Transient hepatitis and cholestatic jaundice
  • Fever, abdominal pain, superinfection, renal dysfunction, toxic nephropathy
  • Hemorrhage, elevated LDH, pancytopenia

Pregnancy & Lactation

  • Use during pregnancy only if clearly needed
  • Small amounts detected in mother's milk
  • Effect on nursing infants not known
  • Caution for nursing women

Precautions & Warnings

  • Prescribing in absence of proven or strongly suspected bacterial infection unlikely to provide benefit
  • Prolonged use may result in overgrowth of nonsusceptible organisms
  • Renal excretion inhibited by Probenecid
  • Caution in individuals with a history of gastrointestinal disease
  • Determine hypersensitivity reactions before therapy
  • Administer cautiously to penicillin-sensitive patients

Overdose Effects

  • Nausea, vomiting, epigastric distress, and diarrhea
