TelAmlo 5 mg+40 mg (Tablet)

Indications

• Treatment of hypertension
• Use as initial therapy in patients who need multiple drugs to achieve blood pressure goals

Pharmacology

• Combination of Telmisartan and Amiodipine
• Telmisartan: angiotensin receptor blocker (ARB), selective angiotensin II antagonist
• Amiodipine: dihydropyridine calcium-channel blocker (CCB), peripheral arterial vasodilator

Dosage & Administration

• Initial dose: 40/5 mg once daily
• Starting larger blood pressure reductions with 80/5 mg once daily
• Add-on therapy for patients not adequately controlled with other medications
• Replacement therapy for patients receiving components separately
• Maximum recommended dose: 80/10 mg once daily

Interaction

• No clinically significant interactions with specific drugs observed
• Telmisartan not metabolized by the cytochrome P450 system
• Amiodipine safely administered with various other drugs

Contraindications

• Known hypersensitivity to the product or its components
• Pregnancy & lactation
• Biliary obstructive disorders, severe hepatic impairment, hypotension, cardiogenic shock, left ventricle outflow tract obstruction

Pregnancy & Lactation

• Pregnancy Categories C (first trimester) and D (second and third trimesters)
• Excretion in human milk unknown
• Discontinue nursing or drug after considering importance to the mother

Precautions & Warnings

• Avoid fetal or neonatal exposure
• Correct volume or salt depletion before initiating therapy
• Monitor for worsening heart failure
• Avoid concomitant use of ACE inhibitor and angiotensin receptor blocker
• Titrate slowly in patients with hepatic or severe renal impairment

Use in Special Populations

• Pediatric use not established
• Initial therapy not recommended in patients ≥75 years old or with hepatic impairment

Overdose Effects

• Telmisartan: hypotension, dizziness, tachycardia, bradycardia
• Amiodipine: excessive peripheral vasodilation, marked hypotension, reflex tachycardia
