Medicine Details

Indications

  • Treatment of upper respiratory tract infections (URTI)
  • Treatment of lower respiratory tract infections
  • Treatment of urinary tract infections (UTI)
  • Bactericidal activity against Gram-positive organisms
  • Bactericidal activity against Gram-negative organisms

Pharmacology

  • Third generation semisynthetic cephalosporin antibiotic
  • Bactericidal to both gram positive and gram negative bacteria
  • Interference in the synthesis of bacterial cell wall
  • High stability in the presence of Beta-lactamase enzyme
  • About 40% to 50% absorption whether administered with or without food

Dosage & Administration

  • Usual course of treatment is 7 days
  • Recommended adults dosage is 200-400 mg daily
  • Recommended children dosage is 8 mg/kg/day
  • Recommended dosage for children in typhoid is 5 mg/kg body weight twice daily for 10-14 days
  • Dosage in renal impairment

Interaction

  • Increases in prothrombin times noted in some patients

Contraindications

  • Known hypersensitivity to cephalosporin antibiotics

Side Effects

  • Diarrhoea and stool changes
  • Nausea, abdominal pain, dyspepsia, vomiting, flatulence
  • Pseudomembraneous colitis
  • Headache and dizziness
  • Allergies including rash, pruritis, urticaria, drug fever, arthralgia
  • Thrombocytopenia, leukopenia, eosinophilia
  • Mild transient change in liver and renal function tests
  • Genital pruritis and vaginitis

Pregnancy & Lactation

  • No adequate and well-controlled studies in pregnant women
  • Caution advised when administered to a nursing woman

Precautions & Warnings

  • Caution advised for patients with hypersensitivity to other drugs
  • Caution advised for penicillin-sensitive patients
  • Caution advised for patients with markedly impaired renal function

Overdose Effects

  • Gastric Lavage may be indicated
  • No specific antidote exists
  • Afixime not removed in significant quantities by dialysis
  • Adverse reactions observed in healthy adult volunteers receiving single doses up to 2 g
