Indications
- Treatment of upper respiratory tract infections
- Treatment of lower respiratory tract infections
- Treatment of urinary tract infections
- Demonstrated efficacy against specific pathogens
- Bactericidal activity against a wide variety of organisms
Pharmacology
- Third generation semisynthetic cephalosporin antibiotic
- Bactericidal against broad spectrum of gram positive and gram negative bacteria
- Interferes with synthesis of bacterial cell wall
- High stability in the presence of Beta-lactamase enzyme
- Approximately 40-50% absorption with or without food
Dosage & Administration
- Usual course of treatment is 7 days, extendable to 14 days if required
- Recommended adult dosage of 200-400 mg daily
- Dosing adjustment for children based on age and weight
- Special dosage regimen for typhoid treatment
- Caution and dosage adjustment in patients with severe renal impairment
Interaction
- Possible increase in prothrombin times in some patients
- Careful consideration in patients receiving anticoagulant therapy
Contraindications
- Hypersensitivity to cephalosporin antibiotics
Side Effects
- Mild and self-limiting adverse reactions observed in clinical trials
- Frequent gastrointestinal disturbances such as diarrhea and stool changes
- Less frequent gastrointestinal side-effects including nausea, abdominal pain, and vomiting
- Other potential side-effects related to central nervous system, hypersensitivity reactions, and hematological parameters
- Mild transient change in liver and renal function tests
Pregnancy & Lactation
- Limited well-controlled studies on pregnant women
- Caution advised for use during pregnancy only if clearly needed
- Unknown excretion in human milk, caution advised for nursing women
Precautions & Warnings
- Caution in patients with known drug hypersensitivity
- Consideration of cross-allergenicity with penicillins, especially in penicillin-sensitive patients
- Administration with caution in patients with markedly impaired renal function
- Alteration of normal flora of the colon and potential risk of antibiotic-associated diarrhoea
- Restriction in use during pregnancy and lactation
Overdose Effects
- Potential need for gastric lavage in case of overdose
- No specific antidote available
- Limited removal from circulation by hemodialysis or peritoneal dialysis
- Inconclusive adverse reactions in healthy volunteers receiving single doses up to 2 g
