Indications

  • Treatment of upper respiratory tract infections (URTI)
  • Treatment of lower respiratory tract infections
  • Treatment of urinary tract infections (UTI)
  • Efficacy against Streptococcus pneumonia
  • Efficacy against Escherichia coli
  • Efficacy against Haemophilus influenzae
  • Efficacy against Proteus mirabilis
  • High stability in the presence of beta-lactamase enzymes

Pharmacology

  • Third generation semisynthetic cephalosporin antibiotic
  • Bactericidal against a broad spectrum of gram positive and gram negative bacteria
  • Interferes with the synthesis of bacterial cell wall
  • High stability in the presence of beta-lactamase enzyme
  • About 40% to 50% absorption whether administered with or without food

Dosage & Administration

  • Usual course of treatment is 7 days, up to 14 days if required
  • Recommended adult dosage: 200-400 mg daily according to severity of infection
  • Recommended dosage for children: 8 mg/kg/day
  • Dosage adjustment in severe renal impairment
  • Recommended dose in typhoid: 5 mg/kg body weight twice daily for 10-14 days
  • Safety and efficacy not established in children aged less than 6 months

Interaction

  • Increases in prothrombin times noted in some patients

Contraindications

  • Hypersensitivity to cephalosporin antibiotics

Side Effects

  • Mild and self-limiting adverse reactions
  • Most frequent side-effects: diarrhea and stool changes
  • Other gastrointestinal side-effects: nausea, abdominal pain, vomiting
  • Central nervous system side-effects: headache and dizziness
  • Hypersensitivity reactions: rash, pruritis, urticaria, drug fever
  • Hematological and clinical chemistry reactions: thrombocytopenia, leukopenia
  • Mild transient change in liver and renal function tests
  • Miscellaneous: genital pruritis, vaginitis

Pregnancy & Lactation

  • No adequate and well-controlled studies in pregnant women
  • Use during pregnancy only if clearly needed
  • Caution advised when administered to a nursing woman

Precautions & Warnings

  • Given with caution to patients with known hypersensitivity to other drugs
  • Caution advised for penicillin-sensitive patients
  • Administered with caution in patients with markedly impaired renal function

Overdose Effects

  • Gastric Lavage may be indicated
  • No specific antidote exists
  • Cefixime is not removed in significant quantities by dialysis
