Indications
- Treatment of upper respiratory tract infections (URTI)
- Treatment of lower respiratory tract infections
- Treatment of urinary tract infections (UTI)
- Clinical efficacy against Streptococcus pneumonia
- Clinical efficacy against Escherichia coli
- Treatment of infections caused by Enterobacter species
- High stability in presence of beta-lactamase enzymes
Pharmacology
- Third generation semisynthetic cephalosporin antibiotic
- Bactericidal against broad spectrum of gram positive and gram negative bacteria
- Interferes in the synthesis of bacterial cell wall
- Highly stable in the presence of beta-lactamase enzyme
- Absorption of about 40% to 50% with or without food
Dosage & Administration
- Usual course of treatment is 7 days, can be continued up to 14 days if required
- Recommended adult dosage: 200-400 mg daily
- Recommended child dosage: 8 mg/kg/day
- Dosage adjustments for elderly and patients with renal impairment
Interaction
- Increases in prothrombin times noted in some patients
- Careful use with anticoagulant therapy
Contraindications
- Known hypersensitivity to cephalosporin antibiotics
Side Effects
- Well tolerated
- Mild and self-limiting adverse reactions
- Frequent gastrointestinal disturbances: diarrhea, stool changes
- Less frequent gastrointestinal side-effects: nausea, abdominal pain, vomiting
- Central nervous system effects: headache, dizziness
- Hypersensitivity reactions: rash, pruritis, urticaria
- Hematological and clinical chemistry reactions: thrombocytopenia, leukopenia, eosinophilia
- Mild transient change in liver and renal function tests
- Other possible reactions: genital pruritis, vaginitis
Pregnancy & Lactation
- Limited studies in pregnant women, cautious use during pregnancy
- Unknown excretion in human milk, caution during lactation
Precautions & Warnings
- Caution in patients with hypersensitivity to other drugs
- Caution in penicillin-sensitive patients
- Markedly impaired renal function
- Alteration of normal flora of the colon
- Use in pregnancy and lactation only when essential
Overdose Effects
- Gastric lavage may be indicated
- No specific antidote exists
- No significant removal by hemodialysis or peritoneal dialysis
- Adverse reactions similar to those at recommended doses
