Medicine Details

Indications

  • Treatment of upper respiratory tract infections (URTI)
  • Treatment of lower respiratory tract infections
  • Treatment of urinary tract infections
  • Marked in-vitro bactericidal activity against a wide range of organisms

Pharmacology

  • Bactericidal against a broad spectrum of gram-positive and gram-negative bacteria
  • Interferes with the synthesis of bacterial cell wall
  • High stability in the presence of beta-lactamase enzyme
  • About 40%-50% absorption with or without food
  • Kills bacteria by interfering in the synthesis of bacterial cell wall

Dosage & Administration

  • Usual course of treatment is 7 days, may be continued for up to 14 days if required
  • Recommended adult dosage is 200-400 mg daily for severe infections
  • Recommended children dosage is 8 mg/kg/day for general infections
  • Dosage adjustment in severe renal impairment
  • Dosage for typhoid in children is 5 mg/kg body weight twice daily for 10-14 days

Interaction

  • Increases in prothrombin times noted in a few patients
  • Care to be taken in patients receiving anticoagulant therapy

Contraindications

  • Hypersensitivity to cephalosporin antibiotics

Side Effects

  • Mild and self-limiting adverse reactions observed in clinical trials
  • Frequent side effects include diarrhea, stool changes, nausea, abdominal pain, dyspepsia, vomiting, flatulence
  • Other possible reactions include headache, dizziness, rash, pruritus, urticaria, drug fever, arthralgia, thrombocytopenia, leukopenia, eosinophilia

Pregnancy & Lactation

  • No adequate and well-controlled studies in pregnant women
  • Caution advised in use during pregnancy and nursing

Precautions & Warnings

  • Caution in patients with hypersensitivity to other drugs
  • Cephalosporin to be given with caution to penicillin-sensitive patients
  • Caution in patients with markedly impaired renal function
  • Alters normal flora of the colon, may permit clostridia overgrowth
  • Not recommended for use in pregnancy or nursing mothers unless considered essential

Overdose Effects

  • Gastric Lavage may be indicated
  • No specific antidote exists
  • No removal in significant quantities by hemodialysis or peritoneal dialysis
