Description

  • Orally active cephalosporin antibiotic
  • Bactericidal activity against a wide variety of Gram-positive and Gram-negative organisms
  • Indicated for upper respiratory tract infections (URTI) and lower respiratory tract infections
  • Clinical efficacy demonstrated against pathogens including Streptococcus pneumonia, Streptococcus pyogenes, Escherichia coli, Proteus mirabilis, Klebsiella species, Haemophilus influenzae, Moraxella catarrhalis, and Enterobacter species
  • Highly stable in the presence of beta-lactamase enzymes
  • Bactericidal action interferes with the synthesis of bacterial cell wall
  • Available in 200-400 mg dosage for adults, 8 mg/kg/day dosage for children, and 5 mg/kg body weight for typhoid treatment
  • Absorption not significantly modified by the presence of food
  • Increases in prothrombin times noted in some patients
  • Contraindicated in patients with known hypersensitivity to cephalosporin antibiotics
  • Generally well tolerated, with mild and self-limiting adverse reactions
  • Should be used during pregnancy only if clearly needed; caution should be exercised when administered to nursing women

Pharmacology

  • Third generation semisynthetic cephalosporin antibiotic
  • Bactericidal against a broad spectrum of gram positive and gram negative bacteria
  • Absorption of about 40% to 50% whether administered with or without food

Dosage administration

  • Usual course of treatment is 7 days, extendable to 14 days if required
  • Recommended adult dosage: 200-400 mg daily; dosage for children: 8 mg/kg/day
  • Lower doses for children aged 6 months to 10 years as per suggested volumes of suspension
  • Special dose for typhoid treatment and caution in renal impairment

Interaction

  • Increases in prothrombin times noted in some patients

Contraindications

  • Known hypersensitivity to cephalosporin antibiotics

Side effects

  • Mild and self-limiting adverse reactions observed in clinical trials
  • Most frequent side-effects: diarrhea and stool changes; less frequent side-effects: nausea, abdominal pain, dyspepsia, vomiting, flatulence
  • Other side-effects include headache, dizziness, allergies in the form of rash, pruritus, urticaria, drug fever, arthralgia, thrombocytopenia, leukopenia, eosinophilia, mild transient change in liver and renal function tests, genital pruritus, vaginitis

Pregnancy lactation

  • No adequate and well-controlled studies in pregnant women; use during pregnancy only if clearly needed
  • Caution advised when administered to nursing women; excretion in human milk not known

Precautions warnings

  • Given with caution to patients who have shown hypersensitivity to other drugs
  • Caution advised in penicillin-sensitive patients and in patients with markedly impaired renal function
  • Alters the normal flora of the colon and may permit overgrowth of clostridia
  • Not to be used in pregnancy or in nursing mothers unless considered essential by the physician

Overdose effects

  • Gastric Lavage may be indicated in case of overdose
  • No specific antidote exists; not removed in significant quantities from the circulation by hemodialysis or peritoneal dialysis
  • Adverse reactions in small numbers of healthy adult volunteers receiving single doses up to 2 g did not differ from the profile seen in patients treated at recommended doses
