Indications

  • Treatment of Upper Respiratory Tract Infections
  • Treatment of Lower Respiratory Tract Infections
  • Treatment of Urinary Tract Infections
  • Effective against Streptococcus pneumoniae
  • Effective against Escherichia coli
  • High stability in the presence of beta-lactamase enzymes

Pharmacology

  • Third generation semisynthetic cephalosporin antibiotic
  • Bactericidal against a broad spectrum of gram positive and gram negative bacteria
  • Kills bacteria by interfering in the synthesis of bacterial cell wall
  • High stability in the presence of Beta-lactamase enzyme
  • About 40%-50% absorption with or without food

Dosage & Administration

  • Usual course of treatment is 7 days, can be continued for up to 14 days
  • Adult dosage: 200-400 mg daily, single or divided doses
  • Recommended dosage for children: 8 mg/kg/day
  • Special dosage for typhoid infection in children
  • Dosing adjustments for elderly and patients with renal impairment

Interaction

  • Possible increases in prothrombin times in some patients
  • Caution needed in patients receiving anticoagulant therapy

Contraindications

  • Hypersensitivity to cephalosporin antibiotics

Side Effects

  • Mild and self-limiting adverse reactions observed in clinical trials
  • Gastrointestinal disturbances like diarrhea and stool changes
  • Central nervous system effects: headache, dizziness
  • Hypersensitivity reactions: rash, pruritis, urticaria

  • Caution in patients with hypersensitivity to other drugs
  • Cephalosporin should be given cautiously to penicillin-sensitive patients
  • Caution in patients with markedly impaired renal function
  • Alteration of normal colon flora and potential for antibiotic-associated diarrhoea
  • Should not be used in pregnancy or nursing mothers unless essential

  • Potential need for gastric lavage in cases of overdose
  • No specific antidote available
  • Adverse reactions similar to those at recommended doses
