Indications

  • Treatment of upper respiratory tract infections
  • Treatment of lower respiratory tract infections
  • Treatment of urinary tract infections
  • Effective against Streptococcus pneumonia
  • Effective against Streptococcus pyogenes
  • Effective against Escherichia coli
  • Effective against Proteus mirabilis
  • Effective against Klebsiella species
  • Effective against Haemophilus influenzae
  • High stability in the presence of beta-lactamase enzymes

Pharmacology

  • Third generation semisynthetic cephalosporin antibiotic
  • Bactericidal against a broad spectrum of gram-positive and gram-negative bacteria
  • Interferes in the synthesis of bacterial cell wall
  • High stability in the presence of beta-lactamase enzyme
  • About 40-50% absorption rate with or without food

Dosage & Administration

  • Usual course of treatment is 7 days
  • May be continued for up to 14 days if required
  • Recommended adult dosage range: 200-400 mg daily
  • Recommended dosage for children: 8 mg/kg/day
  • Individualized dosages for different age groups and conditions
  • Special dosage for typhoid
  • Dosage adjustments for renal impairment

Interaction

  • Potential increases in prothrombin times in some patients

Contraindications

  • Hypersensitivity to cephalosporin antibiotics

Side Effects

  • Mild and self-limiting adverse reactions
  • Gastrointestinal disturbances
  • Central nervous system effects: headache and dizziness
  • Hypersensitivity reactions: rash, pruritis, urticaria
  • Hematological and clinical chemistry effects
  • Other possible reactions include genital pruritis and vaginitis

Pregnancy & Lactation

  • No adequate and well-controlled studies in pregnant women
  • Cautious use during pregnancy if clearly needed
  • Caution exercised when administered to nursing women

Precautions & Warnings

  • Caution in patients with known hypersensitivity to other drugs
  • Partial cross-allergenicity between penicillins and cephalosporins
  • Caution in patients with markedly impaired renal function
  • Potential for antibiotic-associated diarrhoea

Overdose Effects

  • Gastric Lavage may be indicated
  • No specific antidote exists
  • Low removal from circulation by hemodialysis or peritoneal dialysis
  • Adverse reactions in small numbers of healthy volunteers receiving high doses
