Pharmacological profile

  • Semisynthetic cephalosporin antibiotic
  • Bactericidal against broad spectrum of gram positive and gram negative bacteria
  • Interferes with the synthesis of bacterial cell wall
  • Highly stable in the presence of Beta-lactamase enzyme
  • Approximately 40% to 50% absorption with or without food

Dosage and administration

  • Usual course of treatment is 7 days, may be continued up to 14 days if required
  • Adult dosage: 200-400 mg daily according to the severity of the infection, given either as a single dose or in two divided doses
  • Dosage for children based on age and body weight
  • Dosage adjustment required in severe renal impairment

Indications

  • Treatment of upper respiratory tract infections (URTI)
  • Treatment of lower respiratory tract infections
  • Treatment of urinary tract infections

Contraindications

  • Hypersensitivity to cephalosporin antibiotics

Interaction

  • Potential to increase prothrombin times in some patients

Side effects

  • Mild and self-limiting gastrointestinal disturbances
  • Occasional central nervous system effects
  • Possible hypersensitivity reactions
  • Reports of hematological and clinical chemistry changes
  • Mild transient change in liver and renal function tests
  • Occasional genital pruritis and vaginitis

Pregnancy and lactation

  • No adequate and well-controlled studies in pregnant women
  • Caution should be exercised during pregnancy and when administered to nursing women

Precautions and warnings

  • Caution in patients with hypersensitivity to other drugs
  • Caution in penicillin-sensitive patients
  • Administration with caution in patients with markedly impaired renal function
  • Normal flora alteration and potential for antibiotic-associated diarrhea

Overdose effects

  • Indications for gastric lavage
  • No specific antidote exists
  • Not removed significantly by hemodialysis or peritoneal dialysis
  • Adverse reactions in healthy adult volunteers receiving single doses up to 2 g did not differ significantly from recommended doses
