Indications

  • Orally active cephalosporin antibiotic
  • Treatment of upper respiratory tract infections
  • Treatment of lower respiratory tract infections
  • Treatment of urinary tract infections
  • Clinical efficacy against various pathogens
  • Marked in-vitro bactericidal activity

Pharmacology

  • Third generation semisynthetic cephalosporin antibiotic
  • Bactericidal against broad spectrum of bacteria
  • Kills bacteria by interfering in cell wall synthesis
  • High stability in the presence of beta-lactamase enzyme
  • Approximately 40%-50% absorption with or without food

Dosage & Administration

  • Usual course of treatment is 7 days
  • Recommended adult dosage is 200-400 mg daily
  • Recommended dosage for children is 8 mg/kg/day
  • Dosage in typhoid and for children weighing more than 50 kg
  • Adjustment in severe renal impairment
  • Dosage in renal impairment
  • No specific dosage for children aged less than 6 months

Interaction

  • Possible increases in prothrombin times
  • Caution in patients receiving anticoagulant therapy

Contraindications

  • Hypersensitivity to cephalosporin antibiotics

Side Effects

  • Mainly gastrointestinal disturbances
  • Other side effects include headache, dizziness, and hypersensitivity reactions
  • Infrequent hematological and clinical chemistry effects
  • Mild transient change in liver and renal function tests

Pregnancy & Lactation

  • No adequate and well-controlled studies in pregnant women
  • Caution in use during pregnancy
  • Excretion in human milk unknown
  • Caution in nursing women

Precautions & Warnings

  • Caution in patients with known drug hypersensitivity
  • Partial cross-allergenicity between penicillins and cephalosporins
  • Caution in penicillin-sensitive patients
  • Caution in patients with markedly impaired renal function
  • Alteration of normal flora of the colon with broad spectrum antibiotics
  • Avoid use in pregnancy or nursing unless essential

Overdose Effects

  • Gastric Lavage may be indicated
  • No specific antidote exists
  • No significant removal by hemodialysis or peritoneal dialysis
  • Adverse reactions at high doses similar to recommended doses
