Medicine Details

Indications

  • Treatment of upper respiratory tract infections (URTI)
  • Treatment of lower respiratory tract infections
  • Treatment of urinary tract infections (UTI) such as cystitis, cystourethritis, pyelonephritis
  • Effective against Streptococcus pneumoniae
  • Effective against Streptococcus pyogenes
  • Effective against Escherichia coli
  • Effective against Proteus mirabilis
  • Effective against Klebsiella species
  • Effective against Haemophilus influenzae (beta-lactamase positive and negative)
  • Effective against Moraxella catarrhalis (beta-lactamase positive and negative)
  • Highly stable in the presence of beta-lactamase enzymes

Pharmacology

  • Bactericidal against a broad spectrum of gram-positive and gram-negative bacteria
  • Interferes with the synthesis of bacterial cell wall
  • Highly stable in the presence of Beta-lactamase enzyme
  • Absorption is about 40% to 50% whether administered with or without food

Dosage & Administration

  • Usual course of treatment is 7 days
  • Recommended adult dosage is 200-400 mg daily
  • Dose can be given as a single dose or in two divided doses
  • Dosage for children is 8 mg/kg/day
  • Different daily doses for children based on age and weight
  • Recommended children dose for typhoid is 5 mg/kg body weight twice daily for 10-14 days
  • Special precautions for dosage in renal impairment
  • No established safety and efficacy for children aged less than 6 months

Interaction

  • Increases in prothrombin times have been noted in a few patients
  • Care should be taken in patients receiving anticoagulant therapy

Contraindications

  • Known hypersensitivity to cephalosporin antibiotics

Side Effects

  • Most adverse reactions observed in clinical trials were mild and self-limiting
  • Common gastrointestinal disturbances such as diarrhea and stool changes
  • Other gastrointestinal side effects like nausea, abdominal pain, dyspepsia
  • Central nervous system side effects including headache and dizziness
  • Hypersensitivity reactions in the form of rash, pruritus, urticaria
  • Hematological and clinical chemistry disturbances such as thrombocytopenia, leukopenia
  • Mild transient change in liver and renal function tests
  • Other possible reactions include genital pruritus and vaginitis

Pregnancy & Lactation

  • No adequate and well-controlled studies in pregnant women
  • Use during pregnancy only if clearly needed
  • Caution should be exercised when administered to a nursing woman

Precautions & Warnings

  • Given with caution to patients showing hypersensitivity to other drugs
  • Caution advised for penicillin-sensitive patients
  • Administration with caution in patients with markedly impaired renal function
  • Treatment alters normal flora of the colon and may permit overgrowth of clostridia
  • Not recommended in pregnancy or nursing mothers unless essential

Overdose Effects

  • Gastric Lavage may be indicated
  • No specific antidote exists
  • Not removed in significant quantities by hemodialysis or peritoneal dialysis
