Indications

  • Marked in-vitro bactericidal activity against a wide variety of Gram-positive and Gram-negative organisms
  • Treatment of upper respiratory tract infections
  • Treatment of lower respiratory tract infections
  • Treatment of urinary tract infections
  • Clinical efficacy against various pathogens including Streptococcus pneumonia, Escherichia coli, Haemophilus influenzae, etc.

Pharmacology

  • Third generation semisynthetic cephalosporin antibiotic for oral administration
  • Bactericidal against a broad spectrum of gram-positive and gram-negative bacteria
  • Interferes in the synthesis of bacterial cell wall
  • Highly stable in the presence of Beta-lactamase enzyme
  • Approximately 40% to 50% absorption rate whether administered with or without food

Dosage & Administration

  • Usual course of treatment is 7 days, which may be extended to 14 days if required
  • Recommended adult dosage is 200-400 mg daily for upper respiratory tract infections
  • Recommended dosage for children is 8 mg/kg/day
  • Specific daily doses for children based on age and weight
  • Caution and dosage adjustment in severe renal impairment
  • Dose and regimen for patients maintained on chronic ambulatory peritoneal dialysis or hemodialysis

Interaction

  • Potential increases in prothrombin times in some patients receiving anticoagulant therapy

Contraindications

  • Contraindicated in patients with known hypersensitivity to cephalosporin antibiotics

Side Effects

  • Commonly observed side effects including gastrointestinal disturbances, central nervous system effects, hypersensitivity reactions, hematological and clinical chemistry effects, mild transient changes in liver and renal function tests
  • Pseudomembraneous colitis has been reported
  • Thrombocytopenia, leukopenia, and eosinophilia have been reported
  • Genital pruritis and vaginitis
  • Additional miscellaneous reactions

Pregnancy & Lactation

  • No adequate and well-controlled studies in pregnant women
  • Use during pregnancy only if clearly needed
  • Caution in administration to nursing women

Precautions & Warnings

  • Caution in administering to patients with hypersensitivity to other drugs
  • Caution in penicillin-sensitive patients due to partial cross-allergenicity
  • Administration with caution in patients with markedly impaired renal function
  • Alteration of normal flora of the colon and potential overgrowth of clostridia due to treatment with broad-spectrum antibiotics

Overdose Effects

  • Gastric Lavage may be indicated in situations of overdose
  • No specific antidote exists
  • Not significantly removed from circulation by hemodialysis or peritoneal dialysis
