Indications

  • Treatment of upper respiratory tract infections (URTI)
  • Treatment of lower respiratory tract infections
  • Treatment of urinary tract infections
  • Effective against a wide variety of Gram-positive and Gram-negative organisms
  • Marked in-vitro bactericidal activity

Pharmacology

  • Third generation semisynthetic cephalosporin antibiotic
  • Bactericidal against a broad spectrum of gram-positive and gram-negative bacteria
  • Interferes in the synthesis of bacterial cell wall
  • Highly stable in the presence of Beta-lactamase enzyme

Dosage & Administration

  • Usual course of treatment is 7 days
  • Recommended dosage for adults 200-400 mg daily
  • Dosage adjustments for elderly and children based on severity of infection
  • Special dosage recommendation for typhoid
  • Dosage adjustments in renal impairment

Interaction

  • Increases in prothrombin times have been noted in a few patients
  • Caution required in patients receiving anticoagulant therapy

Contraindications

  • Patients with known hypersensitivity to cephalosporin antibiotics

Side Effects

  • Generally well tolerated
  • Most adverse reactions are mild and self-limiting
  • Common gastrointestinal disturbances like diarrhea and stool changes
  • Other side effects include nausea, abdominal pain, headache, and dizziness

Pregnancy & Lactation

  • No adequate and well-controlled studies in pregnant women
  • Use during pregnancy only if clearly needed
  • Caution when administered to nursing women as excretion in human milk is unknown

Precautions & Warnings

  • Caution in patients with hypersensitivity to other drugs
  • Cross-allergenicity between penicillins and cephalosporins
  • Caution in patients with markedly impaired renal function
  • Treatment alters normal flora of colon and may lead to antibiotic-associated diarrhea

Overdose Effects

  • Gastric Lavage may be indicated
  • No specific antidote exists
  • Not removed significantly by hemodialysis or peritoneal dialysis
