Description
Cefotaxime is indicated for the treatment of various infections caused by bacteria of established sensitivity. It is a 3rd generation parenteral cephalosporin antibiotic with broad-spectrum bactericidal activity.

Pharmacology
Cefotaxime binds to penicillin binding proteins, inhibiting peptidoglycan synthesis and resulting in bacterial cell death. It is exceptionally active against gram-negative organisms and has activity against gram-positive bacteria.
Dosage and administration
Adults: 1 gm every 12 hours for mild to moderate infections. Children: 100-150 mg/kg/day in 2 to 4 divided doses. Neonates: 50 mg/kg/day in 2 to 4 divided doses. Gonorrhoea: 500 mg as a single dose.
Interaction
Increased nephrotoxicity may occur when cephalosporins are administered concomitantly with aminoglycoside antibiotics.
Contraindications
Cefotaxime is contraindicated in patients with known hypersensitivity to cefotaxime or cephalosporin antibiotics.

Pregnancy and lactation
The safety of Cefotaxime in human pregnancy has not been established. It is excreted in breast milk.
Precautions and warnings
Prescribe with caution in individuals with a history of gastrointestinal disease, particularly colitis. Dosage adjustment is necessary in patients with renal impairment.
Special populations
- Renal impairment: Dosage adjustment is necessary in severe renal failure. Further adjustment may be required in all other patients according to the course of infection.

Storage conditions
Store below 25°C, protected from light and moisture. Reconstituted solution is stable for up to 24 hours if stored between 2°C to 8°C.
