Description

  • Broad-spectrum bactericidal 3rd generation parenteral cephalosporin antibiotic
  • Indicated for various infections such as respiratory tract, urinary tract, soft-tissue, bone and joint, obstetric and gynaecological, gonorrhoea, and prophylaxis
  • Pharmacologically inhibits bacterial cell wall assembly leading to cell death
  • Dosage varies for adults, children, neonates, and in gonorrhoea
  • Interaction with aminoglycoside antibiotics may increase nephrotoxicity
  • Contraindicated in patients with hypersensitivity to cefotaxime or cephalosporin antibiotics
  • Mild and transient side effects reported including candidiasis, rashes, fever, liver enzyme changes, diarrhea, pseudomembranous colitis, and renal function changes
  • Use with caution in pregnancy and lactation, and in individuals with gastrointestinal disease or renal impairment
  • Therapeutic class: Third generation Cephalosporins
  • Storage conditions: Below 25°C, protected from light and moisture, reconstituted solution stable for up to 24 hours if stored between 2°C to 8°C

Pharmacology

  • Binds to penicillin binding proteins to inhibit bacterial cell wall synthesis

Dosage and Administration for Adults

  • Recommended dosage for mild to moderate infections: 1 gm every 12 hours
  • Dosage may vary according to severity of infection, causative organisms' sensitivity, and patient's condition
  • Severe infections: dosage may be increased up to 12 gm daily in 3 or 4 divided doses
  • Greater than 6 gm daily doses may be required for infections caused by sensitive Pseudomonas spp.

Dosage and Administration for Children

  • Usual dosage range: 100-150 mg/kg/day in 2 to 4 divided doses
  • Up to 200 mg/kg/day doses may be required in very severe infections

Dosage and Administration for Neonates

  • Recommended dosage: 50 mg/kg/day in 2 to 4 divided doses
  • In severe infections: 150-200 mg/kg/day in divided doses

Dosage in Gonorrhoea

  • 500 mg as a single dose

Interaction

  • Concomitant administration with aminoglycoside antibiotics may increase nephrotoxicity

Contraindications

  • Hypersensitivity to cefotaxime or cephalosporin antibiotics

Side Effects

  • Relatively infrequent and mild adverse reactions reported
  • Effects include candidiasis, rashes, fever, liver enzyme changes, diarrhea, pseudomembranous colitis, renal function changes, and hypersensitivity reactions such as skin rashes, drug fever, and anaphylaxis

Pregnancy and Lactation

  • Safety in human pregnancy not established
  • Excreted in the milk

Precautions and Warnings

  • Prescribed with caution in individuals with a history of gastrointestinal disease, particularly colitis
  • Dosage reduction needed in patients with transient or persistent reduction of urinary output due to renal insufficiency
  • No clinical evidence supporting the necessity of changing the dosage in patients with renal dysfunction

Use in Special Populations

  • Dosage reduction necessary in severe renal failure
  • Adjustment of dosage may be required according to the course of infection and the general condition of the patient

Storage Conditions

  • Store below 25°C, protected from light and moisture
  • Reconstituted solution stable for up to 24 hours if stored between 2°C to 8°C
