Indications

  • Treatment of acute otitis media caused by Streptococcus pneumoniae
  • Treatment of pharyngitis/tonsillitis caused by Streptococcus pyogenes
  • Treatment of acute maxillary sinusitis caused by Haemophilus influenzae
  • Treatment of community-acquired pneumonia caused by S. pneumoniae or H. influenza
  • Treatment of acute bacterial exacerbation of chronic bronchitis caused by S. pneumoniae
  • Treatment of skin and skin structure infections caused by Staphylococcus aureus
  • Uncomplicated urinary tract infections caused by E. coli
  • Uncomplicated gonorrhea caused by Neisseria gonorrhoeae
  • Rectal gonococcal infections in women due to Neisseria gonorrhoeae

Pharmacology

  • Oral 3rd generation cephalosporin
  • Good stability to beta lactamases
  • Activity against Gram negative and Gram positive bacteria
  • Prodrug with active metabolite Cefpodoxime
  • Approximately 29 to 33% excreted unchanged in the urine in 12 hours

Dosage & Administration

  • Adults and Adolescents (13 years and older):
    • Different dosages for different conditions
  • Infants and Pediatric Patients (2 months to 12 years):
    • Different dosages for different conditions
  • Patients with renal dysfunction: Dosing intervals should be increased
  • Patients with liver cirrhosis: Dose adjustment not necessary

Interaction

  • Concomitant administration of high doses of antacids or H 2 blockers reduce peak plasma level
  • Renal excretion of cefpodoxime inhibited by probenecid
  • Close monitoring of renal function advised when administered with compounds of known nephrotoxic potential

Contraindications

Known allergy to Cefpodoxime or cephalosporin group of antibiotics

Pregnancy & Lactation

  • Neither teratogenic nor embryocidal in animal trial
  • Should be used during pregnancy only if clearly needed
  • Decision should be made whether to discontinue breast-feeding or to discontinue the drug

Precautions & Warnings

  • In patients with transient or persistent reduction in urinary output due to renal insufficiency, the total daily dose should be reduced
  • Administered with caution to patients receiving concurrent treatment with potent diuretics
  • Prolonged use may result in overgrowth of non-susceptible organisms
