Indications

  • Treatment of acute otitis media caused by various bacteria

  • Treatment of uncomplicated gonorrhea

Pharmacology

  • Oral 3rd generation cephalosporin
  • Good stability to beta-lactamases
  • Activity against Gram-negative and Gram-positive bacteria
  • Prodrug with active metabolite Cefpodoxime
  • Approximately 29-33% excretion unchanged in urine in 12 hours
  • Highly stable in the presence of beta-lactamase enzyme

Dosage & Administration

  • Specific dosages for various infections in adults, adolescents, infants, and pediatric patients
  • Dosing adjustments for patients with renal dysfunction
  • No dose adjustment necessary in patients with liver cirrhosis

Interaction

  • Interaction with antacids and H 2 blockers
  • Inhibition of renal excretion by probenecid
  • Close monitoring of renal function advised for nephrotoxic drugs

Contraindications

  • Contraindicated in patients with known allergy to Cefpodoxime or cephalosporin antibiotics

Side Effects

  • Side effects include diarrhea, nausea, fungal infections, abdominal pain, headache, chest pain, myalgia, dyspepsia, dizziness, vertigo, cough
  • Higher incidence of fungal skin rash in children

Pregnancy & Lactation

  • Not teratogenic or embryocidal in animal trials
  • No adequate and well-controlled study in pregnant women
  • Use during pregnancy only if clearly needed
  • Excretion in human milk, decision required for breast-feeding

Precautions & Warnings

  • Reduction of total daily dose in patients with transient or persistent reduction in urinary output
  • Cautious administration with potent diuretics
  • Possible overgrowth of non-susceptible organisms with prolonged use
