Indications

  • Treatment of acute bacterial exacerbation of chronic bronchitis

  • Treatment of uncomplicated urinary tract infections
  • Treatment of uncomplicated gonorrhea
  • Treatment of rectal gonococcal infections in women

Pharmacology

  • Oral 3rd generation cephalosporin
  • Good stability to beta lactamases
  • Activity against Gram-negative and Gram-positive bacteria
  • Prodrug with active metabolite Cefpodoxime
  • Approximately 29 to 33% excreted unchanged in urine

Dosage & Administration

  • Specific dosages for adults and adolescents
  • Specific dosages for infants and pediatric patients
  • Dosage adjustment for renal dysfunction
  • No dose adjustment necessary for patients with liver cirrhosis

Interaction

  • Interaction with antacids
  • Interaction with probenecid leading to increased AUC
  • Close monitoring advised with nephrotoxic drugs

Contraindications

  • Contraindicated in patients with known allergy to Cefpodoxime or cephalosporins

Side Effects

  • Common side effects include diarrhea, nausea, fungal infections, abdominal pain, headache, and chest pain
  • Higher incidence of fungal skin rash in children

Pregnancy & Lactation

  • Not teratogenic or embryocidal in animal trials
  • Use during pregnancy only if clearly needed
  • Consideration needed for breastfeeding mothers

  • Dosage reduction in patients with reduced urinary output
  • Caution with concurrent use of potent diuretics
