Dosage
- Adult dosage: 1 gram every 8 to 12 hours
- Dosage determined by susceptibility, infection severity, renal function
- Dosage range for adults: 1 to 6 gm per day
- Dosage reductions for elderly patients
- Dosage range for children: 30 to 100 mg/kg/day
- Dosage for neonates and children up to 2 months: 25 to 60 mg/kg/day
Administration
