Dosage

Usual adult dosage: 1 gram every 8 to 12 hours
Dependent on severity of infection and renal function
Dosage range for adults: 1 to 6 gm per day
Dosage range for children: 30 to 100 mg/kg/day
Usual dosage for neonates and children up to 2 months: 25 to 60 mg/kg/day

Administration

Intravenous or deep intramuscular injection
Reconstitution with Sterile Water for Injection
Avoid intra-arterial administration
