Indications

  • Lower respiratory tract infections
  • Acute Bacterial Otitis Media
  • Skin and skin structure infections
  • Urinary tract infections
  • Gonorrhea
  • Bacterial Septicemia
  • Bone and joint infections
  • Meningitis
  • Prevention of postoperative infections
  • Perioperative prophylaxis of infections associated with surgery

Pharmacology

  • Broad-spectrum parenteral cephalosporin antibiotic
  • Potent bactericidal activity
  • Interferes with bacterial cell wall synthesis
  • Stable in the presence of beta lactamases
  • Long plasma elimination half-life (6-9 hours)
  • High degree of protein binding (95%)
  • Not metabolized in the body
  • Excreted unchanged in urine and bile

Dosage

  • Usual adult dose: 1 to 2 gm by intravenous or intramuscular administration once a day
  • Maximum adult dose: 4 gm/day
  • Infants and children dose: 50 to 75 mg/kg intravenous or intramuscular administration once a day
  • Maximum infants and children dose: 2 gm/day
  • Duration of therapy: 4 to 14 days

  • Intramuscular and intravenous injection
  • Preparation of solutions for injections
  • Administered over 2-4 minutes
  • Patient tolerance test recommended before treatment

Contraindications

  • Hypersensitivity to cephalosporin antibiotics

Side Effects

  • Gastro-intestinal effects (diarrhea, nausea, vomiting)
  • Cutaneous reactions (rash, pruritus, urticaria)
  • Hematologic reactions (eosinophilia, thrombocytopenia)
  • Hepatic reactions (elevations of SGOT or SGPT, bilirubinemia)
  • CNS reactions (nervousness, confusion, headache)
  • Local phlebitis

Pregnancy & Lactation

  • Safety not established in human pregnancy
  • Caution during lactation

Precautions & Warnings

  • Potential for anaphylactic shock
  • Gallbladder sludge (reversible on discontinuation)
  • Regular blood checks during prolonged treatment

Use in Special Populations

  • Not for premature and newborn neonates (up to 28 days of age)

Overdose Effects

  • No specific antidote
  • Symptomatic treatment
