Indications

  • Lower respiratory tract infections
  • Acute Bacterial Otitis Media
  • Skin and skin structure infections
  • Urinary tract infections
  • Gonorrhea
  • Bacterial Septicemia
  • Bone and joint infections
  • Meningitis
  • Prevention of postoperative infections
  • Perioperative prophylaxis of infections associated with surgery

Pharmacology

  • Broad-spectrum parenteral cephalosporin antibiotic
  • Bactericidal activity against a wide range of Gram-positive and Gram-negative organisms
  • Interferes with the synthesis of the bacterial cell wall
  • High stability in the presence of beta lactamases
  • Relatively long plasma elimination half-life (6 to 9 hours)
  • Not metabolized in the body
  • Highly protein bound (95%)

Dosage

  • Usual adult dose: 1 to 2 gm by intravenous or intramuscular administration once a day (or in equally divided doses twice a day)
  • Maximum dose for adults: 4 gm/day
  • Usual infant and children dose: 50 to 75 mg/kg intravenous or intramuscular administration once a day (or in equally divided doses twice a day)
  • Maximum dose for infants and children: 2 gm/day

Administration

  • Preparation of Solutions for Intramuscular / Intravenous Injections
  • Injected over 2-4 minutes intramuscularly or intravenously

Contraindications

  • Hypersensitivity to cephalosporin antibiotics

Side Effects

  • Gastro-intestinal effects (diarrhea, nausea, vomiting)
  • Cutaneous reactions (rash, pruritus, urticaria)
  • Hematologic reactions (eosinophilia, thrombocytopenia, leucopenia)
  • Hepatic reactions (elevations of SGOT or SGPT, bilirubinemia)
  • CNS reactions (nervousness, confusion, headache)
  • Local phlebitis

Pregnancy & Lactation

  • Safety during pregnancy not established. Caution in lactating mothers

Precautions & Warnings

  • Possible anaphylactic shock
  • Shadow suggesting sludge in gallbladder detected in rare cases
  • Regular blood picture check during prolonged treatment

Use in Special Populations

  • Neonates if premature and newborn (up to 28 days of age)

Overdose Effects

  • No specific antidote. Symptomatic treatment recommended
