Indications

  • Lower respiratory tract infections
  • Acute Bacterial Otitis Media
  • Skin and skin structure infections
  • Urinary tract infections
  • Gonorrhea
  • Bacterial Septicemia
  • Bone and joint infections
  • Meningitis
  • Prevention of postoperative infections
  • Perioperative prophylaxis of infections associated with surgery

Pharmacology

  • Broad-spectrum parenteral cephalosporin antibiotic
  • Bactericidal activity against Gram-positive and Gram-negative organisms
  • Interferes with the synthesis of the bacterial cell wall
  • High degree of stability in the presence of beta lactamases
  • Relatively long plasma elimination half-life of 6 to 9 hours
  • Not metabolized in the body
  • Highly protein bound (95%)

Dosage

  • Usual dose of 1 to 2 gm by intravenous or intramuscular administration once a day
  • Different doses for various infections in adults and infants/children
  • Maximum dose of 4 gm/day for certain infections
  • Duration of therapy is 4 to 14 days

Administration

  • Preparation and dissolution for intramuscular/intravenous injections
  • Administration over 2-4 minutes
  • Test dose recommended before starting treatment
  • Potency maintained for at least 6 hours at room temperature or 24 hours at 5°C

  • No reported drug interactions

Contraindications

  • Not to be given to patients with a history of hypersensitivity to cephalosporin antibiotics

Side Effects

  • Gastro-intestinal effects including diarrhea, nausea and vomiting
  • Cutaneous reactions including rash, pruritus, urticaria
  • Hematologic reactions including eosinophilia, thrombocytopenia
  • Hepatic reactions including elevations of SGOT or SGPT, bilirubinemia
  • CNS reactions including nervousness, confusion, headache
  • Local phlebitis occurs rarely following intravenous administration

Pregnancy & Lactation

  • Safety in human pregnancy not established
  • Caution advised when administered to lactating mother

Precautions & Warnings

  • Potential for anaphylactic shock
  • Occasional detection of shadows in sonograms of the gallbladder
  • Regular blood picture checks during prolonged treatment

Use in Special Populations

  • Not to be given to neonates if premature and newborn (up to 28 days of age)

Overdose Effects

  • No specific antidote
  • Treatment should be symptomatic
