Indications
- Lower respiratory tract infections

Pharmacology
- Broad-spectrum parenteral cephalosporin antibiotic
- Bactericidal activity against wide range of Gram-positive and Gram-negative organisms
- Interferes with synthesis of bacterial cell wall
- High stability in presence of beta lactamases
- Long plasma elimination half-life of 6 to 9 hours
- Not metabolized in the body
- Highly protein bound (95%)
- Excreted in urine and bile/feces
Dosage
- Usual adult dose: 1 to 2 gm by intravenous or intramuscular administration once a day
- Maximum adult dose: 4 gm/day
- Usual pediatric dose: 50 to 75 mg/kg intravenous or intramuscular administration once a day
- Maximum pediatric dose: 2 gm/day
- Duration of therapy: 4 to 14 days, longer in complicated infections
Administration
- Preparation of solutions for intramuscular/intravenous injections
- Recommended injection over 2-4 minutes
- Freshly reconstituted solution recommended

Contraindications
- History of hypersensitivity to cephalosporin antibiotics
Side Effects
- Gastro-intestinal effects including diarrhea, nausea and vomiting
- Cutaneous reactions including rash, pruritus, urticaria
- Hematologic reactions including eosinophilia, thrombocytopenia
- Hepatic reactions including elevations of SGOT or SGPT, bilirubinemia
- CNS reactions including nervousness, confusion, sleep disturbances
- Local phlebitis following intravenous administration
Pregnancy & Lactation
- Safety in human pregnancy not established
- Low concentrations excreted in human milk
Precautions & Warnings
- Risk of anaphylactic shock
- Potential gallbladder sludge
- Regular blood checks during prolonged treatment
Use in Special Populations
- Avoid in neonates if premature or newborn (up to 28 days of age)
