Indications

  • Lower respiratory tract infections
  • Acute Bacterial Otitis Media
  • Skin and skin structure infections
  • Urinary tract infections
  • Gonorrhea
  • Bacterial Septicemia
  • Bone and joint infections
  • Meningitis
  • Prevention of postoperative infections
  • Perioperative prophylaxis of infections associated with surgery

Pharmacology

  • Broad-spectrum antibiotic
  • Bactericidal activity
  • Targets Gram-positive and Gram-negative organisms
  • Interferes with bacterial cell wall synthesis
  • High stability in the presence of beta lactamases
  • Relatively long plasma elimination half-life
  • Not metabolized in the body
  • Highly protein bound

Dosage

  • Usual adult dose of 1 to 2 gm
  • Maximum adult dose of 4 gm/day
  • 250 mg IM dose for uncomplicated gonococcal infections
  • 1 g IV dose for surgical prophylaxis
  • Usual infant and children dose of 50 to 75 mg/kg
  • Maximum infant and children dose of 2 gm/day
  • Duration of therapy of 4 to 14 days

Administration

  • Intramuscular / Intravenous Injection preparation guidelines
  • Dissolution in Lidocaine HCI 1% for intramuscular injection
  • Dissolution in Water for injection for intravenous injection
  • Injection administered over 2-4 minutes
  • Patient tolerance test recommended

  • No reported drug interactions

Contraindications

  • Hypersensitivity to cephalosporin antibiotics

Side Effects

  • Gastro-intestinal effects including diarrhea, nausea and vomiting
  • Cutaneous reactions including rash, pruritus, urticaria
  • Hematologic reactions including eosinophilia, thrombocytopenia
  • Hepatic reactions including elevations of SGOT or SGPT
  • CNS reactions including nervousness, confusion, sleep disturbances
  • Local phlebitis following intravenous administration

Pregnancy & Lactation

  • Safety in human pregnancy not established
  • Caution in lactating mothers

Precautions & Warnings

  • Potential for anaphylactic shock
  • Sonogram findings related to gallbladder
  • Regular blood picture checks during prolonged treatment

Use in Special Populations

  • Avoid in premature and newborn neonates (up to 28 days of age)

Overdose Effects

  • No specific antidote
  • Symptomatic treatment
