Medicine Details

Indications

  • Lower respiratory tract infections
  • Acute Bacterial Otitis Media
  • Skin and skin structure infections
  • Urinary tract infections
  • Gonorrhea
  • Bacterial Septicemia
  • Bone and joint infections
  • Meningitis
  • Prevention of postoperative infections
  • Perioperative prophylaxis of infections associated with surgery

Pharmacology

  • Broad-spectrum parenteral cephalosporin antibiotic
  • Bactericidal activity
  • Interferes with bacterial cell wall synthesis
  • Stable in the presence of beta lactamases
  • Long plasma elimination half-life (6 to 9 hours)
  • Not metabolized in the body
  • Excreted mostly unchanged in urine and feces
  • Highly protein bound (95%)

Dosage

  • Adult dose of 1 to 2 gm by intravenous or intramuscular administration once a day
  • Maximum adult dose of 4 gm/day
  • Infants and Children dose of 50 to 75 mg/kg intravenous or intramuscular administration once a day
  • Maximum infant and children dose of 2 gm/day
  • Duration of therapy usually 4 to 14 days

Administration

  • Preparation of Solutions for Intramuscular / Intravenous Injections
  • Dissolving in Lidocaine HCI 1% injection or Water for injection
  • Administered over 2-4 minutes for intramuscular or intravenous injection
  • Administered via tubing infusion over a period of 30 minutes
  • Patient tolerance test recommended before treatment

Contraindications

  • History of hypersensitivity to cephalosporin antibiotics

Side Effects

  • Gastro-intestinal effects including diarrhea, nausea, vomiting, stomatitis and glossitis
  • Cutaneous reactions including rash, pruritus, urticaria, edema and erythema multiforme
  • Hematologic reactions including eosinophilia, thrombocytopenia, leucopenia, anemia and neutropenia
  • Hepatic reactions including elevations of SGOT or SGPT, bilirubinemia
  • CNS reactions including nervousness, confusion, sleep disturbances, headache, hyperactivity, convulsion, hypertonia and dizziness
  • Rare occurrence of local phlebitis following intravenous administration

Pregnancy & Lactation

  • Safety in human pregnancy not established
  • Caution advised in lactating mothers

Precautions & Warnings

  • Risk of anaphylactic shock
  • Potential for sludge in the gallbladder
  • Regular blood picture checks during prolonged treatment

Use in Special Populations

  • Neonatal use not advisable for premature and newborn (up to 28 days of age)

Overdose Effects

  • No specific antidote
  • Symptomatic treatment advised
