Indications
- Lower respiratory tract infections

Pharmacology
- Broad-spectrum parenteral cephalosporin antibiotic
- Bactericidal activity
- Interferes with bacterial cell wall synthesis
- Stable in the presence of beta lactamases
- Long plasma elimination half-life (6 to 9 hours)
- Not metabolized in the body
- Excreted mostly unchanged in urine and feces
- Highly protein bound (95%)
Dosage
- Adult dose of 1 to 2 gm by intravenous or intramuscular administration once a day
- Maximum adult dose of 4 gm/day
- Infants and Children dose of 50 to 75 mg/kg intravenous or intramuscular administration once a day
- Maximum infant and children dose of 2 gm/day
- Duration of therapy usually 4 to 14 days
Administration
- Preparation of Solutions for Intramuscular / Intravenous Injections
- Dissolving in Lidocaine HCI 1% injection or Water for injection
- Administered over 2-4 minutes for intramuscular or intravenous injection
- Administered via tubing infusion over a period of 30 minutes
- Patient tolerance test recommended before treatment

Contraindications
- History of hypersensitivity to cephalosporin antibiotics
Side Effects
- Gastro-intestinal effects including diarrhea, nausea, vomiting, stomatitis and glossitis
- Cutaneous reactions including rash, pruritus, urticaria, edema and erythema multiforme
- Hematologic reactions including eosinophilia, thrombocytopenia, leucopenia, anemia and neutropenia
- Hepatic reactions including elevations of SGOT or SGPT, bilirubinemia
- CNS reactions including nervousness, confusion, sleep disturbances, headache, hyperactivity, convulsion, hypertonia and dizziness
- Rare occurrence of local phlebitis following intravenous administration
Pregnancy & Lactation
- Safety in human pregnancy not established
- Caution advised in lactating mothers
Precautions & Warnings
- Risk of anaphylactic shock
- Potential for sludge in the gallbladder
- Regular blood picture checks during prolonged treatment
Use in Special Populations
- Neonatal use not advisable for premature and newborn (up to 28 days of age)
