Indications
- Lower respiratory tract infections

Pharmacology
- Broad-spectrum parenteral cephalosporin antibiotic
- Bactericidal activity against wide range of Gram-positive and Gram-negative organisms
- Interferes with synthesis of bacterial cell wall
- High stability in presence of beta lactamases
- Relatively long plasma elimination half-life of 6 to 9 hours
- Not metabolized in the body
- Highly protein bound (95%)
Dosage
- Usual adult dose of 1 to 2 gm intravenously or intramuscularly once a day
- Maximum adult dose of 4 gm/day
- 250 mg intramuscular dose for uncomplicated gonococcal infections
- Infant and child doses of 50 to 75 mg/kg intravenously or intramuscularly once a day
- Maximum infant and child dose of 2 gm/day
- Duration of therapy usually 4 to 14 days
Administration
- Preparation of solutions for intramuscular / intravenous injections
- Intramuscular injection preparation in Lidocaine HCI 1% injection
- Intravenous injection preparation in Water for injection
- Administration over 2-4 minutes for intramuscular or intravenous injection or by tubing infusion over 30 minutes
- Patient tolerance test recommended before treatment
- Solution potency maintained for at least 6 hours at room temperature or 24 hours at 5°C

Contraindications
- History of hypersensitivity to cephalosporin antibiotics
Side Effects
- Gastro-intestinal effects including diarrhea, nausea, and vomiting
- Cutaneous reactions including rash, pruritus, and urticaria
- Hematologic reactions including eosinophilia and thrombocytopenia
- Hepatic reactions including elevations of SGOT or SGPT
- CNS reactions including nervousness, confusion, and sleep disturbances
- Rare local phlebitis following intravenous administration
Pregnancy & Lactation
- Safety in human pregnancy not established
- Use in pregnancy only if absolutely indicated
- Low concentrations excreted in human milk
- Caution during lactation
Precautions & Warnings
- Potential for anaphylactic shock
- Rare cases of gallbladder sludge detected
- Blood picture should be checked during prolonged treatment
Use in Special Populations
- Not to be given to premature and newborn neonates

- Symptomatic treatment recommended
