Indications

  • Lower respiratory tract infections
  • Acute Bacterial Otitis Media
  • Skin and skin structure infections
  • Urinary tract infections
  • Gonorrhea
  • Bacterial Septicemia
  • Bone and joint infections
  • Meningitis
  • Prevention of postoperative infections
  • Perioperative prophylaxis of infections associated with surgery

Pharmacology

  • Broad-spectrum parenteral cephalosporin antibiotic
  • Bactericidal activity against wide range of Gram-positive and Gram-negative organisms
  • Interferes with synthesis of bacterial cell wall
  • High stability in presence of beta lactamases
  • Relatively long plasma elimination half-life of 6 to 9 hours
  • Not metabolized in the body
  • Highly protein bound (95%)

Dosage

  • Usual adult dose of 1 to 2 gm intravenously or intramuscularly once a day
  • Maximum adult dose of 4 gm/day
  • 250 mg intramuscular dose for uncomplicated gonococcal infections
  • Infant and child doses of 50 to 75 mg/kg intravenously or intramuscularly once a day
  • Maximum infant and child dose of 2 gm/day
  • Duration of therapy usually 4 to 14 days

Administration

  • Preparation of solutions for intramuscular / intravenous injections
  • Intramuscular injection preparation in Lidocaine HCI 1% injection
  • Intravenous injection preparation in Water for injection
  • Administration over 2-4 minutes for intramuscular or intravenous injection or by tubing infusion over 30 minutes
  • Patient tolerance test recommended before treatment
  • Solution potency maintained for at least 6 hours at room temperature or 24 hours at 5°C

Contraindications

  • History of hypersensitivity to cephalosporin antibiotics

Side Effects

  • Gastro-intestinal effects including diarrhea, nausea, and vomiting
  • Cutaneous reactions including rash, pruritus, and urticaria
  • Hematologic reactions including eosinophilia and thrombocytopenia
  • Hepatic reactions including elevations of SGOT or SGPT
  • CNS reactions including nervousness, confusion, and sleep disturbances
  • Rare local phlebitis following intravenous administration

Pregnancy & Lactation

  • Safety in human pregnancy not established
  • Use in pregnancy only if absolutely indicated
  • Low concentrations excreted in human milk
  • Caution during lactation

Precautions & Warnings

  • Potential for anaphylactic shock
  • Rare cases of gallbladder sludge detected
  • Blood picture should be checked during prolonged treatment

Use in Special Populations

  • Not to be given to premature and newborn neonates

Overdose Effects

  • No specific antidote
  • Symptomatic treatment recommended
