Indications

  • Lower respiratory tract infections
  • Acute Bacterial Otitis Media
  • Skin and skin structure infections
  • Urinary tract infections
  • Gonorrhea
  • Bacterial Septicemia
  • Bone and joint infections
  • Meningitis
  • Prevention of postoperative infections
  • Perioperative prophylaxis of infections associated with surgery

Pharmacology

  • Broad-spectrum parenteral cephalosporin antibiotic
  • Bactericidal activity against Gram-positive and Gram-negative organisms
  • Interferes with bacterial cell wall synthesis
  • High stability in the presence of beta lactamases
  • Long plasma elimination half-life of 6 to 9 hours
  • Not metabolized in the body
  • Excretion in urine and feces
  • Highly protein bound (95%)

Dosage

  • Usual adult dose of 1 to 2 gm by intravenous or intramuscular administration once a day
  • Maximum dose of 4 gm/day for certain infections
  • Usual pediatric dose of 50 to 75 mg/kg intravenous or intramuscular administration once a day
  • Maximum pediatric dose of 2 gm/day for certain infections
  • Duration of therapy: 4 to 14 days

Administration

  • Preparation of solutions for intramuscular / intravenous injections
  • Injection administered over 2-4 minutes
  • Patient tolerance test recommended
  • Potency maintained for at least 6 hours at room temperature or 24 hours at 5°C

Contraindications

  • Not to be given to patients with hypersensitivity to cephalosporin antibiotics

Side Effects

  • Gastro-intestinal effects including diarrhea, nausea, and vomiting
  • Cutaneous reactions including rash, pruritus, and urticaria
  • Hematologic reactions including eosinophilia, thrombocytopenia, and anemia
  • Hepatic reactions including elevations of liver enzymes
  • CNS reactions including nervousness, confusion, and sleep disturbances
  • Local phlebitis following intravenous administration

Pregnancy & Lactation

  • Safety in human pregnancy not established
  • Caution in lactating mothers due to low concentrations excreted in human milk

Precautions & Warnings

  • Risk of anaphylactic shock
  • Potential gallbladder sludge
  • Regular blood picture check during prolonged treatment

Use in Special Populations

  • Not to be given to neonates if premature and newborn (up to 28 days of age)

Overdose Effects

  • No specific antidote
  • Symptomatic treatment recommended
