Indications

  • Lower respiratory tract infections
  • Acute Bacterial Otitis Media
  • Skin and skin structure infections
  • Urinary tract infections
  • Gonorrhea
  • Bacterial Septicemia
  • Bone and joint infections
  • Meningitis
  • Prevention of postoperative infections
  • Perioperative prophylaxis of infections associated with surgery

Pharmacology

  • Bactericidal activity against a wide range of Gram-positive and Gram-negative organisms
  • Interferes with the synthesis of the bacterial cell wall
  • High stability in the presence of beta lactamases
  • Long plasma elimination half-life (6 to 9 hours)
  • Not metabolized in the body
  • High degree of protein binding (95%)
  • Excretion in urine and bile

Dosage

  • Usual dose for adults (1 to 2 gm by intravenous or intramuscular administration once a day)
  • Dosing based on infection type
  • Specific doses for uncomplicated gonococcal infections and surgical prophylaxis
  • Usual dose for infants and children (50 to 75 mg/kg intravenous or intramuscular administration once a day)

Contraindications

  • Not to be given to patients with a history of hypersensitivity to cephalosporin antibiotics

  • Hematologic reactions (eosinophilia, thrombocytopenia, etc.)
  • Hepatic reactions (elevations of SGOT or SGPT, bilirubinemia)
  • CNS reactions (nervousness, confusion, sleep disturbances, etc)
  • Local phlebitis following intravenous administration

Pregnancy & Lactation

  • Safety in human pregnancy not established
  • Caution advised when administered to lactating mothers

Precautions & Warnings

  • Potential for anaphylactic shock
  • Rare cases of gallbladder shadows detected
  • Recommend blood picture checks during prolonged treatment

Use in Special Populations

  • Must not be given to neonates if premature and newborn (up to 28 days of age)

Overdose Effects

  • No specific antidote
  • Treatment should be symptomatic
