Indications

  • Essential hypertension
  • Angina pectoris with hypertension
  • Post myocardial infarction

Pharmacology

  • Fixed-dose combination of Amlodipine and Atenolol
  • Amlodipine - dihydropyridine calcium antagonist
  • Amlodipine - peripheral vasodilator
  • Amlodipine - reduces peripheral vascular resistance and blood pressure
  • Amlodipine - reduces coronary vasoreactivity and lowers cardiac demand
  • Atenolol - cardioselective beta-blocker
  • Atenolol - reduces blood pressure by lowering cardiac output and plasma renin activity
  • Atenolol - reduces myocardial oxygen demand

Dosage & Administration

  • Recommended dosage: Amlodipine and Atenolol 5/25 mg tablet once daily
  • Dosage may be increased to 5/25 mg two tablets daily under physician's advice
  • Dosage should be individualized

Interactions

  • Disopyramide - Atenolol reduces clearance, producing negative inotropic effects
  • Ampicillin - may reduce Atenolol levels
  • Oral antidiabetics and insulin - Beta-blockers may decrease tissue sensitivity to insulin and inhibit insulin secretion

Contraindications

  • Hypersensitivity to either component
  • Sinus bradycardia
  • Second and higher degrees of heart block
  • Cardiogenic shock
  • Hypotension
  • Congestive heart failure
  • Poor left ventricular function

Side Effects

  • Well tolerated overall
  • Possible side-effects: fatigue, headache, edema, nausea, drowsiness, anxiety, depression

Pregnancy & Lactation

  • Use during pregnancy if benefit outweighs potential fetal risk
  • Not for use by nursing mothers

Precautions & Warnings

  • Bronchospasm - caution in airway obstruction
  • Renal impairment - caution if creatinine clearance is less than 30 ml/min
  • Hepatic impairment - caution in severe liver damage
  • Drug withdrawal - discontinuation should be gradual and under observation

Overdose Effects

  • Hypotension
  • Congestive cardiac failure
  • Unabsorbed drugs may be removed by gastric lavage or activated charcoal
  • Symptomatic treatment suggested
