Indications

  • Treatment of moderate to severe pneumonia caused by Streptococcus pneumoniae, Pseudomonas aeruginosa, Klebsiella pneumoniae, or Enterobacter species
  • Empiric treatment of febrile neutropenic patients
  • Treatment of uncomplicated and complicated urinary tract infections, including pyelonephritis
  • Treatment of uncomplicated skin and skin structure infections
  • Treatment of complicated intra-abdominal infections (used in combination with metronidazole)

Pharmacology

  • Bactericidal action
  • Disruption of peptidoglycan layer synthesis in bacterial cell walls

Dosage and Administration

  • Administered intravenously over approximately 30 minutes
  • Specific dosage for different types of infections

Interaction

  • Increased potential of nephrotoxicity and ototoxicity when administered with high doses of aminoglycosides
  • Nephrotoxicity reported with concomitant administration of other cephalosporins with potent diuretics

Contraindications

  • Immediate hypersensitivity reactions to cefepime or the cephalosporin class of antibiotics
  • Hypersensitivity reactions to penicillin or other betalactum antibiotics

Side Effects

  • Potential occurrence of hypersensitivity reactions

Pregnancy and Lactation

  • Pregnancy Category B
  • Excretion in breast milk in very low concentrations
  • Caution advised during administration to nursing woman

Precautions and Warnings

  • Avoid prescribing in the absence of proven or strongly suspected bacterial infection
  • Risk of overgrowth of non susceptible microorganisms with prolonged use
  • Association with fall in prothrombin activity
  • Caution in patients with gastrointestinal disease
  • Effect of arginine on glucose metabolism and serum potassium levels

Use in Special Populations

  • Maximum dose for pediatric patients should not exceed recommended adult dose
  • Dosage recommendations for pediatric patients based on weight
  • Serious adverse events reported in geriatric patients with renal insufficiency
  • No adjustment necessary for patients with impaired hepatic function
  • Dose adjustment necessary for patients with impaired renal function

Overdose Effects

  • Careful observation and supportive treatment for patients who receive an overdose
  • Recommended hemodialysis in the presence of renal insufficiency
