Description

  • Merocil is a carbapenem antibiotic for parenteral use, indicated for the treatment of various infections caused by bacteria sensitive to Merocil. It exerts its bactericidal action by interfering with bacterial cell wall synthesis. The recommended daily dosage and duration of therapy vary depending on the type and severity of infection, as well as the patient's condition. Merocil should be administered by intravenous infusion over approximately 15-30 minutes or as an intravenous bolus. It is generally well tolerated but may cause side effects such as inflammation, pain at the injection site, skin reactions, abdominal pain, nausea, vomiting, diarrhea, and headache. Use in special populations, precautions, and overdose effects should be considered. The therapeutic class of Merocil is other beta-lactam antibiotics. It should be stored in a cool, dry place away from light and moisture.

Pharmacology

  • Bactericidal action by interfering with bacterial cell wall synthesis
  • Potent activity against a broad spectrum of Gram-positive and Gram-negative, aerobic and anaerobic bacteria

Dosage

  • Dosage and duration depend on the type and severity of infection, and patient's condition
  • Adults: 500 mg to 1 gm intravenously every 8 hours, varying based on infection type
  • Children: 10 to 40 mg/kg intravenously every 8 hours, with specific dosages for different infections and age groups

Administration

  • Intravenous infusion over approximately 15-30 minutes or as intravenous bolus (5 to 20 ml) over approximately 3-5 minutes

Interaction

  • Probenecid competes with Merocil for active tubular secretion, inhibiting renal excretion and increasing plasma concentration
  • May reduce serum valproic acid levels

Contraindications

  • Hypersensitivity to Merocil

Use in Special Populations

  • Renal impairment: Dosage reduction for creatinine clearance < 51 ml/min
  • Hepatic impairment: No dosage adjustments necessary
  • Elderly: No dosage adjustments unless creatinine clearance is < 51 ml/min

Overdose Effects

  • Accidental overdose more likely in renal impairment
  • Treatment should be symptomatic

Reconstitution

  • Intravenous bolus: Reconstitute with sterile water for injection, clear and colorless or pale yellow solution
