Pharmacology
- Main mechanism of action on protein synthesis
- Energy-dependent active transport system
- Binds specifically to the 30s ribosomes
- Inhibits bacterial protein synthesis
- Susceptible to Gram positive aerobic Cocci
- Resistant strains of staphylococci and streptococci
- Active against Brucella species
- Active against Calymmatobacterium granulomatis
- Active against Vibrio cholerae
- Active against V. vulnificus
- Active against anaerobic species of bacteria
- Incompletely absorbed from the gastrointestinal tract
- Half-life of about 12 hours
Dosage & Administration
- Usual adult oral dosage of 1-2g daily
- Usual oral dosage for children older than 8 years
- Taken preferably one hour before or 2 hours after meals

- Specific dosage for Acute staphylococcal infections
- Specific dosage for Acute streptococcal infections
- Specific dosage for Amoebiasis

Interaction
- Impaired absorption with antacids containing divalent and trivalent cations
- May interfere with the bactericidal action of penicillin
- May potentiate the effect of anticoagulants
- May decrease efficacy of oral contraceptives
- Nephrotoxic effects may be exacerbated by diuretics
- May increase the hypoglycemic effect of insulin
- May increase toxic effects of ergot alkaloids and methotrexate
Contraindications
- Hypersensitivity to any member of tetracycline groups
- Cross-sensitivity in patients with systemic lupus erythematosus
- Contraindicated in renal impairment
- Doses should be reduced in severe renal impairment
Side Effects
- Graybrown discoloration of the teeth
- Enamel hypoplasia
- Hypersensitivity reactions such as anaphylaxis and rashes
- Photosensitivity reactions
- Gastrointestinal effects
- Epigastric distress and nausea
- Accentuated prerenal azotemia
- Superinfections with oral and anogenital candidiasis
- Esophageal ulcerations
Pregnancy & Lactation
- Not to be used during pregnancy
- Potential for impaired bone growth and permanent discoloration of the child's teeth if used during breastfeeding and in children up to the age of 8-12 years
Precautions & Warnings
- Care should be taken if given to patients with impaired liver function
- High doses should be avoided
- Avoid concomitant use with potentiality hepatotoxic drugs
